Overview




Provides case management procedures, therapeutic interventions and clinical care coordination for children, adolescents and their families.

The care management clinician is a valued part of the Psychiatry multidisciplinary team, serving children ranging from preschool to adolescents across several individualized psychiatry clinics. The care management position is rooted in the fundamentals of wrap around services. A care management clinician works collaboratively with internal and external stakeholders to deliver patient care in the outpatient psychiatry setting. Responsibilities include clinical assessments and recommendations, clinical triage, brief clinical intervention, psychoeducation, patient/family engagement, coordination of care, and collaboration.

This unique role has a shared responsibility in a community consultation program with Primary Care Physicians. Early morning start time required.
LISW or LPCC is strongly preferred.

Responsibilities




* Conducts assessments of patients' mental health needs and develops individualized care plans. Monitors patients' progress and adjusts care plans as needed.
* Coordinates client care with other service providers and collaborates with interdisciplinary team to ensure continuity of care.
* Coordinates available NCH and community resources to enhance client care.
* Provides education to patients, families, and care teams about mental health conditions and treatment options.
* Performs brief therapeutic interventions and counseling.
* Completes clinical charting and other related administrative paperwork in accordance with policy standards.
* Participates in quality improvement initiatives to improve patient outcomes.


Qualifications




Education Requirement: Master's degree in Counseling, Social Work or Marriage and Family Therapy, required.

Licensure Requirement:
* Appropriate Ohio licensure, required. Must remain current in required licensure and clinical or other training as required.
* Independent license or satisfactory progress toward independent licensure, required.

Skills: Excellent communication and interpersonal skills in order to establish and maintain positive relationships with clients, families, peers and community representatives.

Experience: Experience providing and managing behavioral healthcare to children, adolescent and families, required.
