Job Description: Access Health Stark County has multiple part-time and full-time Community Health Worker / Case Management positions in Canton, OH.
Training will be provided.
Hybrid schedule offered after training is completed.
The Community Health Worker (CHW) / Case Management is a home visitor who is primarily focused on the client’s health. The CHW connects underserved clients to primary care, health insurance and community resources so the client’s health becomes the priority.
Minimum Requirements:
- Must be over 18 years of age, have valid driver’s license and proof of insurance
- High school diploma or General Education Diploma (GED)
- Must be able to travel all areas of Stark County and have reliable personal vehicle
- Must obtain CHW certification within the first year of employment
- Ability to pass a federal and state background check and drug test
- Must maintain required caseload according to number of hours worked
- Must have knowledge of local resources and, preferably, live in Stark County
Key Responsibilities:
- Establish trusting relationships
- Conduct home-based interviews and enter data into computer
- Help patients set personal goals and attend primary care appointments as needed
- Connect clients to resources in their community (i.e., insurance, medical home, transportation, housing, food, etc.)
- Exhibit excellent working relationships with patients and team members
- Ability to work with others of various ages, backgrounds, cultures and life experiences
- Ability to maintain regular attendance on the job
*** Applicants must have COVID-19 vaccine or an exempt form and TB testing. TB testing will be provided by Access Health.
Details: Part time and full time. First Shift (8:00 a.m. to 4:30 p.m.). 28 to 40 hours/week. Monday through Friday. Starting rate $15.00 to $18.00/hour based on experience. Sign-on retention bonus offered.
Qualifications: High school diploma / GED. Must be at least 18 years of age. Must be able to lift at least 20 lbs.
Driver’s License Requirement(s): Must Have Valid Driver’s License.
