Job Description

Position Title: STNA                                                                        Shifts: 12hr 6a-6p, 8hr 6a-2p, 2p-10p, 10p-6a

Employee Status: Non-Exempt                                                   Reports to: Clinical Manager or Charge Nurse

 

 

Purpose/Summary Statement:

This position is responsible for providing direct care and assistance in activities of daily living to residents under the supervision of a licensed nurse in accordance with facility policies and procedures. The STNA assists residents with dressing, bathing, toileting, oral care, transferring and transporting, eating and nutrition. STNA's provide residents with comfort, assurance and emotional support and are the primary contacts with resident family members and visitors.

 

Essential Tasks and Functions:

  • Makes rounds with outgoing shift; knows whereabouts of assigned residents.
  • Obtains reports from nurse.
  • Obtain and carry Resident Information Sheet in uniform pocket at all times to reference resident's specific needs.
  • Review Resident Information Sheet and perform care consistent with guidelines and directives as contained in the Care Plan.
  • Participate in daily shift huddles.
  • Knows and follows HIPAA regulations
  • Provides morning care, which may include bed bath, shower or whirlpool, oral hygiene, combing hair, back care, dressing residents, changing bed linen, cleaning overbed table and bedside stand, straightening room and other general care as necessary throughout the day.
  • Provides reality orientation in daily care for appropriate residents.
  • Performs all aspects of resident care in an environment that optimizes resident safety and reduces the likelihood of medical/health errors.
  • Restrains residents properly, when ordered.
  • Make sure residents have easy access to call lights and that call lights are functioning properly.
  • Performs general care activities for residents in isolation.
  • Perform simple housekeeping tasks to ensure that resident rooms and common areas are free of trip risks or other safety hazards.
  • Checks unit for adequate linen. Folds neatly and arranges linen in linen closet. Cleans linen cart. Provides clean linen and clothing. Makes beds.
  • Checks for reddened areas or skin breakdown and reports to RN or LPN.
  • Notifies appropriate licensed personnel when resident complains of pain.
  • Reports and, when appropriate, records any changes observed in condition or behavior of residents and unusual incidents including but not limited to changes in food or liquid intake, toileting or vital signs.
  • Has knowledge and ability to use the organization's electronic health record system.
  • Document all care provided in resident record kiosk in a timely manner following facility procedures.
  • Distributes drinking water and other nourishments to residents.
  • Transports residents to and from various departments, as requested.
  • Assists LPN/LVN in treatment procedures.
  • Takes and records temperature, pulse, respiration, weight, blood pressure and intake-output.
  • Prepares residents for meals; serves and removes food trays and assists with meals or feeds residents, if necessary.
  • Participates in socialization activities on the unit.
  • Assist in the preparation for admission of residents.
  • Participates in and contributes to interdisciplinary care conferences.
  • Assists in and accompanies residents in the admission, transfer and discharge procedures.
  • Provides general nursing care such as positioning residents, lifting and turning residents, applying/utilizing special equipment, assisting in the use of bedpan or commode and ambulating the residents.
  • Accompanies residents to appointments, as directed.
  • Makes rounds with incoming shift to ensure the unit is left in good condition.
  • Assists residents with handling and care of clothing and other personal property (including dentures, glasses, contact lenses, hearing aids and prosthetic devices).
  • Provides evening care which includes hands/face washing as needed, oral hygiene, back rubs, peri care, freshening linen, cleaning overbed tables, straightening room and other general care as needed.
  • Ensures residents are dressed properly and assists as necessary, Ensures that used clothing is properly stored in bedside stand or on hangers in closet. Ensures that all residents are clean and dry at all times.
  • Answers residents' call lights, anticipates residents' needs and makes rounds to assigned residents.
  • Turns and positions residents as ordered and/or needed, making sure no rough surfaces are in direct contact with the body. Lifts and turns with proper and safe body mechanics and with available resources.
  • Respond to resident call lights and assist coworkers to ensure resident requests for assistance are answered promptly and thoroughly.
  • Practices careful, efficient and non-wasteful use of supplies and linen and follows established charge procedure for resident charge items.
  • Initiate corrective actions as necessary and/or seek guidance from a licensed nurse.
  • Assist new employees in acclimating to facility policies, procedures and routines.
  • Accept assigned duties in a cooperative manner.

 

Additional Duties:

  • Follow all safety rules, policies and procedures, including building security procedures.
  • Know and follow facility emergency/disaster response and evacuation procedures.
  • Participate in safety events, use assigned safety equipment and PPE and encourage coworkers to work safely.
  • Report observed accidents, injuries or near misses and ensure potentially unsafe conditions are corrected, either by yourself or by a qualified person.

Position-Specific Requirements:

  • Provide assistance in activities of daily living as described in the facilities care policies and procedures.
  • Report changes in resident condition, resident, family and guest concerns to licensed nurse.

 

Qualifications:

  • High School diploma or G.E.D.
  • Possess current certification as a Nurse Aide issued by facility's state. This requirement may be waived for trainees who complete their certification training under Kingston's supervision. However, trainees must complete the training successfully within the timeframes established by the state to retain employment.
  • Current Basic Cardiac Life Support certification within three (3) months of hire date.
  • No past experience required, but experience in Long-Term Care and/or in Senior programming is desired.
  • Must be able to speak, read and understand English, follow written and verbal directions and be able to write legibly. Must have computer skills sufficient to enter and retrieve information from a computerized resident charting program and to take courses on a web-based employee training program.
  • Have a positive attitude toward the elderly.
  • Visual acuity to determine the accuracy, thoroughness and quality of work assigned. Aural acuity to receive detailed information through oral communication, often from persons with speech impairment. Must be able to express or exchange ideas by means of spoken word, often with residents with hearing impairments.
  • Must be able to lift and move 50 pounds frequently, and occasionally bear weight of 100 pounds or more.
