Job Description

Position Title: RN                                                                            

Employee Status: Non-Exempt                                                   Reports to: Clinical Director and DON

 

Purpose/Summary Statement:

The purpose of this position is to supervise, provide and coordinate nursing care in compliance with facility policies and procedures and to assess residents' care needs and take appropriate action.

 

Essential Tasks and Functions:

  • Obtain report from nurse being relieved and record sufficient information to implement appropriate follow-up action as necessary.
  • Initiate Team Huddle if Clinical Director is not available.
  • Take appropriate action to assure adequate staffing to provide resident care.
  • Count narcotics, check emergency kit and exchange keys.
  • Has knowledge of HIPAA regulations.
  • Has knowledge and ability to use the organization's electronic health record system.
  • Conduct resident rounds daily, reporting problems to Clinical Director and initiating corrective action.
  • Follows through, as needed, on information given by shift report, resident or family regarding resident concerns.
  • Consults other departments as appropriate to provide for an interdisciplinary approach to the patient's needs.
  • Identify resident problems and emergency situation and initiate emergency care and "life-saving" measures in the absence of a physician.
  • Provide instructions for appropriate nursing care to licensed nurses and Nursing Assistants.
  • Administers medications, following the five (5) medication rights and reduces the potential for medication errors.
  • Order, receive and store medications appropriately.
  • Ability to perform a head-to-toe assessment on all patients and reassessments as per policy. This includes: geriatric and the general patient population.
  • Ability to adequately assess and reassess pain. Utilizes appropriate pain management techniques. Educates the resident and family regarding pain management.
  • Assesses the resident's condition and nursing needs; sets goals; prescribes appropriate nursing action.
  • Assess residents on admission, during daily routine and when conditions change. Documents appropriately in the Electronic Health Record.
  • Communicates appropriately and clearly to Clinical Director, co-workers, and physicians and the Rehabilitation Services staff.
  • Promptly report changes in residents' conditions to the physician, Director of Nursing Service and responsible party, and take follow-up action as necessary.
  • Documents changes in resident's condition and nursing care on progress notes and plan of care.
  • Notes and carries out physician's orders in a timely manner
  • Implements plan of care for the resident based on assessments and goals as established by the interdisciplinary care team.
  • Ability to revise plan of care as indicated by the patient's response to treatment and evaluate overall plan daily for effectiveness.
  • Formulates a teaching plan based upon identified learning needs and evaluates effectiveness of learning; family is included in teaching as appropriate.
  • Initiates and leads resident care conferences and updates plan of care, as appropriate.
  • Supervise response to resident's call for assistance.
  • Supervise and evaluate all direct resident care and initiate corrective action as necessary.
  • Follows proper infection control procedures and supervises supportive staff in infection control procedures.
  • Ability to perform waived testing (point-of-care testing) per Clinical Laboratory's and the resident care unit's policies and procedures.
  • Ability to interpret results of waived tests; take appropriate action on waived test results.
  • Check meal trays and nourishment to ensure proper diet is served and residents are assisted as necessary.
  • Supervise the application and removal of all restraints and devices used to position and enable residents to function as independently as possible.
  • Follow facility discharge/transfer procedure and coordinate discharge/transfer with other departments, physicians and ancillary service providers.
  • Complete documentation related to the resident's discharge/transfer.
  • Submits electronic/ written documentation in a timely, clear, concise and pertinent manner.
  • Supervises other members of the healthcare team, as needed, to accomplish the plan of care which has been developed.
  • Counsel Nursing Assistants and recommend disciplinary action to the Team Leader/ Director of Nursing, when necessary.
  • Assume responsibility for compliance with federal, state and local regulations within the assigned unit.
  • Participates in orientation of new personnel and performance evaluations of nursing staff.
  • Provide report to nurse coming on duty, including sufficient information for follow-up action as necessary.

 

Employee and Resident Safety:

  • Follow all safety rules, policies and procedures, including building security procedures.
  • Know and follow facility emergency/disaster response and evacuation procedures.
  • Participate in safety events, use assigned safety equipment and PPE and encourage coworkers to work safely.
  • Report observed accidents, injuries or near misses and ensure potentially unsafe conditions are corrected, either by yourself or by a qualified person.

  • Ensure confidentiality, which includes maintaining a workplace free of gossip and rumors.
  • Protect resident rights, report any suspected violations immediately to Management.

 

 

Position-Specific Requirements:

  • Take appropriate action to assure adequate staffing to provide resident care.
  • Supervise and assess resident care and take appropriate action.
  • Implement facility procedures to provide resident care.

 

Qualifications:

  • Must be at least 18 years of age.
  • Graduate of an accredited school of nursing. Currently licensed with the State Board of Nursing as a Registered Nurse or Licensed Practical Nurse in good standing and meeting all applicable federal and state licensure requirements.
  • Previous geriatric nursing experience desirable.
  • Experience in supervising nursing personnel desirable.
  • Must be organized, self-directed and highly motivated with demonstrated leadership and supervisory skills.
  • Must be able to speak, write and understand English. Excellent verbal and written communication skills are necessary.
  • Must have computer skills sufficient to enter and retrieve information from a computerized charting program and take courses on a web based employee training program.
  • Must possess a positive attitude toward the elderly.
  • Finger, hand dexterity, visual and aural acuity is necessary.
  • Must be able to lift 50 pounds regularly.
