  1. Overview of Position

 

Working with the Stark County Jail on their new STAR program to provide aftercare to clients leaving jail. Preform required SUD assessments in person giving a more quickly and smooth path to aftercare. Assisting the current CDCA with needs to assure they can focus on current residential clients. Also, will provide transportation and support directly from jail to services.

 

Essential Job Functions / Responsibilities

 

  1. Improve access to Care.

 

  1. Improve support to jail staff by assessing clients onsite at jail facility

 

  1. Improve privacy/ confidentiality for the inmate with in person interaction.

 

  1. Provides chemical dependency intakes/screenings/assessments, with the assessment summary accurately integrating and summarizing all available data, with Q.A. chart reviews establishing competency.

 

  1. Orients new clients to the agency’s treatment philosophy, hours of operation, service array, client rights/responsibilities, grievance procedure, confidentiality regulations, rules and regulations.

 

  1. Places clients in the most appropriate yet least restrictive treatment modality with access to appropriate intervention services, with Q.A. chart reviews establishing competency.

 

  1. Performs clinical record keeping which accurately documents the findings, treatment, and clinical outcomes on a timely basis for each client served, demonstrating competency by chart reviews, quality assurance reviews, and supervisory reviews.

 

  1. Provides individual counseling, demonstrating competency in individual counseling as evidenced by clinical chart reviews, case consultation, and supervisory observations.

 

  1. Provides group counseling, demonstrating competency in group facilitation, as evidenced by supervisory observations.

 

  1. Provides crisis intervention/counseling for individuals to resolve or stabilize the crisis, establishing competency through incident report reviews, training records and progress notes.

 

  1. Provides client and family education demonstrating competency in clinical progress notes, pre/post test scores, and supervisory observation.

 

  1. Performs clinical record keeping which accurately documents the findings, treatment, and clinical outcomes on a timely basis for each client served, demonstrating competency by chart reviews, quality assurance reviews, and supervisory reviews.

 

  1. Provides case management to assist clients in obtaining needed services and linkages with referral sources, employers, family members, other treatment providers and to assist in the discharge planning process, with competency evidenced by chart reviews, case consultation and supervisory observations.

 

  1. Meets the CommQuest Services productivity standard guidelines.

 

  1. All other duties as assigned.

  6. Promptly reports and documents any critical or unusual incidents.

  7. Performs actions that demonstrate accountability and good judgment in emergency situations.

  8. Demonstrates through behaviors a core belief/value in the fundamental equality of all human beings.

 

  • Global Competency Areas

 

  1. Maintains current certification in CPR/First Aid.

  2. Must be trained and certified in Non-Violent Crisis Intervention training.

  3. Demonstrates knowledge of HIPAA, AoD and Federal Confidentiality Law.

  4. Demonstrates commitment to professional growth as evidenced by attendance at staff meetings and in-services.

  5. Demonstrates ability to recognize possible signs of abuse and neglect of minors as evidenced by reports to supervisors.

  6. Demonstrates knowledge of basic infection control procedures as evidenced in post test score.
  7. Demonstrates competency in responding during agency fire drills, as evidenced in the written reports of actual fire drills or actual fires.

 

  1. Demonstrates understanding of agency disaster protocols.

 

  1. Minimum Qualifications

 

Bachelor’s Degree in counseling, social work or a related field.  Certified/licensed as LCDC II, LCDC III, LICDC, or LICDC-CS.  One year of experience working as a chemical dependency counselor.  If working with adolescents, one year of experience working with adolescents.  Demonstrates knowledge of addiction, Alcoholics Anonymous and other 12-step programs, assessments, treatment plans, and individual and group counseling.  Must be fluent in written and spoken English.
