Who are you?

You are a caring professional who is passionate about providing high quality nursing care, working directly with clients that have a primary mental health diagnosis and/or co-occurring disorders. You are dedicated to making a difference in your local community by focusing on treatment planning and transitions in care to outpatient services, facilitating a seamless coordination with community-based mental health and SUD services.
This is a day shift Monday-Friday position for 10-20 hours/week at Family Resource Center's (FRC) inpatient, residential facility. No weekends or on call hours!! Must be willing to work Monday-Friday.

Under guidance of the Nursing Team Lead, you will assess general health care of clients, observe general physical and emotional condition of clients, check and record vital signs, and any other examinations or treatments that may be necessary. Review problematic behaviors and/or symptoms with clients and/or family. Engage clients in locating appropriate health services and resources (lab work, making initial contact with service provider, making appointment for services), and facilitate any new referrals. Prepare and update medical files of clients, document health problems, and obtain medication consent.

Maintain medical supplies and submit supply recommendations and purchase orders. Document billable units using the agency scheduler and clinical services software programs. Obtain and facilitate the authorization for release of information for Med/Somatic consumers. Perform injections, venipunctures, drug screens, medication education, logging, and oversight. Review and update Individual Service Plans (ISP) and complete the DLA-20 for medical clients.

Working as part of an interdisciplinary team is something you enjoy. Serve as a liaison between medical staff and clients. Review new client health assessment forms and report recommendations for medical examination or further health assessment to the clinical staff. Communicate with pharmacies and assist with medication management.

You have.

  • Valid Licensed Practical Nurse (LPN) license by the State of Ohio
  • Associate's degree or higher in nursing
  • A valid driver's license and the ability to maintain auto insurance
  • CPR/First Aid Certification or the willingness to obtain one, as well as the willingness to submit to background check and fingerprinting
  • Ability to regularly use various computer apps including Microsoft Office, our HRIS, EHR, and other online systems, with multiple logins often simultaneously


Preferred.

  • One-plus (1+) years related experience in a psychiatric or community mental health service
