Responsibilities:

  • Provide direct patient care in accordance with the highest professional standards and practices.
  • Administer medications and treatments as prescribed by healthcare providers.
  • Monitor and assess residents' health and update patient records accordingly.
  • Collaborate with a multidisciplinary team to develop and implement individualized care plans.
  • Ensure a safe and clean environment for residents and adhere to infection control protocols.
  • Communicate effectively with patients, families, and other healthcare team members.

Qualifications:

  • Current LPN license in the state of Ohio.
  • CPR certification.
  • Proven experience in a skilled nursing or long-term care setting is preferred.
  • Strong clinical assessment and critical thinking skills.
  • Excellent communication and interpersonal abilities.
  • Ability to work effectively in a fast-paced environment and adapt to changing priorities.
