PURPOSE OF POSITION:
The coordinator is a member of the inpatient medicine team who is responsible for delivery of processes designed to improve quality and efficiency in the care of hospitalized patients. The nurse works closely with the Inpatient medicine team, which includes hospitalists, specialty hospitalists and neurology as well as the leadership team to engage and educate the physician team, optimize performance on defined key metrics, patient treatment, and ensure collaboration with the care team. The nurse will assist in the coordinator of our neurology inpatient medicine and stroke program. The role will support and implement changes to the management and care of our patients, including the stroke population at Aultman Hospital and support to the Health Delivery System. The coordinator works to achieve and maintain regulatory certification as well as quality measures and regulatory compliance.
SUMMARY:
The inpatient medicine coordinator serves as clinical liaison and collaborates with all disciplines across different levels of the health care continuum and applies the process of assessment, planning, implementation, monitoring, evaluation, and coordination of care to meet the patients' health care needs to ensure safe, quality, and cost-effective care. The RN must provide efficient, quality service. The RN will accomplish this goal by the timely coordination of quality healthcare services to meet an individual's specific health care needs in a cost-effective manner and by providing an excellent patient experience. This position is also responsible for overseeing and facilitating clinical performance activities, potentially across multiple service lines. Activities include identifying cohorts of patients eligible for specific initiatives, identification and analysis of readmissions participating in daily workflows, advocating for patient goals in the care plan, and ensuring communication throughout the care team as the patient moves through their hospital stay. The coordinator will assist leadership to assure standardization of excellence and care for patients, including stroke patients. This care encompasses clinical treatment, decisions and diagnosis for inpatients and assures quality care for the stroke patient throughout the continuum. They will function as a stroke champion to designated stroke units.
RESPONSIBILITIES & EXPECTATIONS
Participate in the development of site-specific workflows to achieve program goals and performance targets, that are aligned with hospital priorities
Provide timely status of implementation, barrier resolution, and action plans through real time chart reviews and abstractions
Understand, clearly articulate, and promote the adoption of hospital and AIM clinical performance objectives with the team
Engage and support education to provider team(s) related to clinical performance initiatives, goals, and measurement
Collaborate with the onsite team to establish and communicate patients' care goals
Adherence to Regulatory body such as The Joint Commission or ACHC or the body chosen by the program/hospital
Maintain core measure data base, including stroke
Participate in OH Coverdell program
CE maintenance for stroke, per requirement of governing body (8 per year specific to stroke)
Preparation and availability for site visits
Stroke committee management and maintenance of data presentations
Monthly stroke alert variance meeting
Stroke Alerts
Attend in person to assist and reduce door to needle time.
Attend LVO to facilitate process and transfer
Communicate with neurologist if not in house
Participate in proactive evaluation of program metrics and initiate additional investigation of drivers when targets are not achieved
Continually evaluate processes, tools, and workflows to identify opportunities for improvement especially with the stroke population
Maintains active communication with the team, displaying a flexible and innovative approach to health management
Promotes and enhances patient -provider communication.
Continues those efforts through active and ongoing communication with next level of care providers, supplying needed information, both written and verbal, to promote positive outcomes
Promotes a positive patient experience and supports initiatives
Rounds on stroke patients within the hospital to ensure patient satisfaction, understanding and excellence in care
Educate patients related to diagnosis: prevention, work up, follow up care
Audits compliance of stroke core measures daily.
Serves as an expert resource in stroke processes and procedures.
Maintenance of continuing education as required by the program
Participates and involves nursing staff in evaluative research activities.
Collaboration with nursing for applicable designated stroke areas
NIH and maintaining core measures
Function as clinical resources for designated stroke unites
Professional leadership and clinical nursing development, resulting in an improved job satisfaction
Disseminate information between coordinator, UDs and staff
Assist with achieving stroke quality measures through implementation of current stroke education on designated units
Assist in resolution of patient centered process issues
Provide peers with resources and concurrent feedback
Assist in data collection and analysis of unit specific quality metrics for staff to review on a quarterly basis
Participate in stroke outreach programs and conferences
Collaboratively work with the neuro-hospitalist department
Patient list maintenance
Assist with order sets and stroke core measures
Hospital - Stroke Champion for Stroke - Collaboration with all units for maintenance, ongoing education of policies and procedures and certification maintenance
Assist with Neuro-hospitalist program, and onboarding of new physicians.
Transitions of care for neuro-hospitalist patients and assisting in discharge follow up visits.
Provides education to hospital staff which includes but is not limited to hospital orientation, Critical Care classes, annual safety education, and presentations given by the stroke medical director.
Serves as expert in stroke care utilizing telemedicine.
Tele-stroke collaboration at main campus and delivery system
Network use of tele-stroke
Collaboration with Entity hospitalist
Stroke Champion resource to entities
Maintain SOC coverage to Aultman network
Collaboration with EMS for stroke training, education and relationships
Maintains continuing education credits as required by licensing/credentialing organization.
Utilizes tracking tool for actions, interventions, and outcomes
Monitors and generates reports on decided dashboards for the department
Actively participates in development initiatives that support the strategic plan for the hospital
Actively participates in performance improvement and quality initiatives as assigned
Participates in regular department meeting forums
Fosters role of advocacy promoting positive care outcomes in fiscally responsible manner
Practices Ethical Conduct:
Demonstrates honesty and integrity in all functions and relationships
Maintains Aultman standards of confidentiality, including HIPAA privacy guidelines
Complies with policies and procedures always
Meets acceptable attendance and punctuality expectations (excluding FMLA)
Organizes job responsibilities and sets priorities based on treatment plan, patient needs, hospital policies and departmental policies.
Anticipates and identifies variances in the care process related to patient needs.
Records pertinent data in patient records in a timely, accurate, clear and concise manner for other team members to provide care/services in an efficient, continuous manner.
Monitors, analyzes, and communicates quality and clinical data.
Other duties may be assigned to meet the needs of the department or organization
Practice Support
Participate in hospital committees and specials projects as appropriate
Meeting with new providers to review role and processes
Collaboration with leadership team to contribute to daily huddles or monthly meetings
Serve as point of contact for hospital staff specific to inpatient neurology
Serve as a point of contact for hospitalists or AIM specialist hospitalists to assist as needed
Management of physician patient assignments specific to neurology
Update Media communications
Additional duties may be assigned to meet the needs of the department/organization/sites
Job Requirements
Graduate of approved School of Nursing, BSN required
3-5 years experience in clinical setting with strong clinical knowledge and experience in the Acute Care Hospital setting (Critical Care, Emergency Department, and/or Case Management experience is helpful)
SCRN certification required within 1 year of meeting eligibility requirements
Current licensure in the State of Ohio
Knowledge of process improvement methodology
Ability to assess patient's clinical status and readiness for discharge
Ability to work both independently and collaboratively
Excellent organizational skills with the ability to multi-task and prioritize workload in a fast-paced environment while maintaining attention to detail and accuracy, and meeting deadlines
Excellent written and oral communication skills, including strong presentation skills
Client service oriented with the ability to interact and communicate with individuals at all levels (e.g., patients, physicians, hospital staff, hospital administration and other colleagues)
Strong interpersonal skills to handle sensitive situations and confidential information. Position continually requires demonstrated poise, tact, diplomacy, and good judgment
Proficiency with technology, including electronic medical systems and Microsoft Office Suite
Knowledge of managed care concepts in relation to financial aspects of patient care including billing, documentation, and coding
Demonstrated ability to handle frequent interruptions in a courteous and professional manner.
Demonstrates ability to utilize information systems within the scope of the position.
Functions independently with flexibility, integrity, and the ability to work effectively and efficiently with all levels of personnel.

WORKING CONDITIONS:
Hours of operation are 7:00 AM to 5:00 PM, Monday through Friday, of which an eight-hour shift is assigned. Off Shift or weekend coverage as indicated to meet the needs of the department and/or organization.
Lunch and break periods must be coordinated with other staff members to maintain adequate office staffing during departmental operating hours.
Sitting/standing/moving about intermittently during working hours.
Subject to frequent interruptions and changes in priority of duties throughout the day.
