JOB SUMMARY

The data collection analyst will assist with a variety of projects related to internal communications and department processes and procedures. Provides assistance with the planning, coordination, and delivery of administrative activities for the department. This includes scheduling, documentation and record keeping of program stats, productivity and benchmarks, maintenance of spreadsheets, program policies and documents, and other support activities for all Population Health Programs. Performs concurrent and retrospective reviews of patient records to abstract data for requested statistics. Compiles and organizes data into meaningful reports for evaluation. Documents accurately and in a timely manner. Deals discretely with sensitive, confidential information. Interacts with others in a respectful and courteous manner. Communicates with other healthcare team members within and outside the department on various program needs. Adheres to organizational policies and procedures.

KNOWLEDGE AND SKILLS

  • General knowledge of health care field including agencies and associations. 
  • Knowledge of current trends in value-based care, care coordination and population health.
  • Working knowledge of organizational development processes and familiarity with hospital documents. 
  • Knowledge of hospital organization and functions.
  • Ability to collect, analyze and report data and other pertinent information.
  • Able to use good judgement, organizationally mature critical thinking,
  • Excellent command of language and communication skills in dealing with all team members, patients, business associates and referral partners.
  • Documentation of meetings and actions through contemporaneous minutes.
  • Demonstration of effective time management, problem-solving, change management, decision-making, conflict resolution, interpersonal and teamwork skills.
  • Strong attention to detail with solid organization and critical thinking skills: planning and tracking, prioritizing, and managing internal and external resources and expectations as well as follow up.
  • Ability to work independently with limited supervision.
  • Strong quantitative, analytical and computer skills.
  • Effective project management skills with ability to manage multiple concurrent tasks and prioritize workflow is essential.
  • Demonstration of positive customer service skill set and positive team-oriented attitude.
  • Successful completion of mandatory requirements including department competencies.

PRIMARY JOB RESPONSIBILITIES

  • Responsible for all data entry of program productivity counts and other information as requested
  • Compiles weekly and monthly statistical information for all Population Health programs
  • Trends and tracks designated data points for various department programs including high risk patients, readmissions, program stats, navigator benchmarks and productivity
  • Maintain files for reports, correspondence and other documents
  • Ensures data integrity, checks all work for accuracy and completeness.
  • Communicates effectively with patients, team members, healthcare providers, business partners and referral sources.
  • Accepts delegated responsibilities on assigned project work.
  • Develops and maintains clinic systems and processes including ordering of office supplies.

  • Assists with control of costs through judicious use of human and materiel resources.

ADDITIONAL RESPONSIBILITIES

  • Attends and prepares minutes of designated meetings.
  • Is knowledgeable about the policies and procedures and governing documents of Knox Community Hospital and the department of Population Health.
  • Serves as necessary to back up clinic PSR and MA duties within scope.
  • Ability to take direction in a professional manner.
  • Prioritizes job duties depending on the work climate.
  • Participates in mandatory in-services, staff meetings, educational programs, committees and QI activities.
  • Establishes and maintains a constructive professional relationship with healthcare team members, business partners and referral sources.
  • Maintains a professional image and abides by dress code.
  • Performs other duties as assigned.

Requirements Include

EDUCATION AND WORK EXPERIENCE

  • Bachelor’s degree in business or health care related field preferred.
  • Excellent computer skills required including proficiency in software programs (Microsoft Word, Microsoft Excel, Microsoft Access, Microsoft Power Point, and other Windows-based programs).
  • Experience in health care preferred.
  • Project management experience preferred.
  • 1-3 years of recent business or customer service-related field beneficial
  • Previous experience as executive assistant to CEO or similar executive level position preferred.
