JOB RESPONSIBILITIES

* Psychosocial Assessment- Performs comprehensive assessments utilizing critical thinking skills to formulate questions that will provide insight into the social and medical factors that influence the patient and family. Provides anticipatory guidance. Demonstrates broad knowledge of abuse, neglect, and other safety risk factors. Report suspicions of abuse and neglect to legally mandated authorities for investigations. Completes documentation with accuracy and clarity.

* Resource Support- Demonstrates ability to make appropriate clinical decisions in high risk situations and facilitate the implementation of appropriate interventions including the engagement on internal and external resources. Maintains fluent knowledge of internal and external resources and individualizes recommendations based on patient and family needs. Provide real time accurate information to patient/family/caregiver for resources for which they are eligible. Provide condition-specific and related medical, financial, educational, and social supportive resource information. Utilizes knowledge and experience to identify opportunities to modify treatment plan to achieve outcomes

* Psychosocial Interventions- Provide psychosocial services as identified in the patient's comprehensive plan of care. Creates opportunities for and provides supportive counseling with the goal of improving emotional coping and adherence to the treatment plan. Initiates collaboration with the referral source and appropriate members of the health care team in a timely and effective manner. Uses advanced knowledge and skill to educate the patient/family/caregiver and members of the health care team about treatment options. Skillfully provides self-management support to high risk/complex patients/families to increase their skills and confidence to effectively manage their chronic care conditions at home. Identifies needs, develops programs, and evaluates outcomes for special populations, as needed. Skillfully provides anticipatory guidance and planning that encourages partnership of patient/families/caregiver in reducing or eliminating psychosocial barriers to discharge. Initiate and facilitate family centered care team meetings. Anticipates points of crisis and intervenes proactively with patients and families to maintain equilibrium.

* Interdisciplinary Collaboration- Working for system improvement, promoting patient/family/caregiver well-being. Helps families become effective consumers of the health care system using knowledge of hospital, division/specialty, and/or community systems Initiates liaison role between the patient/family/caregiver within the medical team and out
