Job Description

Position Title: Care Manager (RN)
Work Location: Remote - OH (West Central / Dayton, OH)
Assignment Duration: 8 months
Work Schedule: 8a-5p, Monday-Friday
Work Arrangement: Remote with field visits

Position Summary:

Required to do field visits (member-facing)

Background & Context:

Two different types of case loads (one working with over 60 waiver population; other will be working with the under 60 population)

Assessments face-to-face visits for maintaining safety welfare; audit their caseloads

Key Responsibilities:

We work out of EMR system has a tasking system where we prioritize our day to day

Group chats daily

Remote so MUST be self-starter

Qualification & Experience:

Required: Associate's degree of Nursing

Required: RN (OH or compact)

Must have Home Health background

Must have Behavioral Health and experience with working with mental health and substance abuse

Must have technology skills (multiple systems)

Must be a self-starter with great time management
