What You'll Do:
- Serve as the primary point of coordination for individuals with developmental disabilities
- Develop personalized support plans using person-centered planning principles that help individuals achieve their goals and dreams
- Assess individual choice, including passions, interests and dreams, as well as identify potential risks
- Connect people with community resources and services that enhance their quality of life
- Collaborate with families, service providers, and community partners to create comprehensive support networks
- Monitor service delivery to guarantee quality and compliance with all regulations
- Maintain detailed case documentation
- Assist with employment services coordination for individuals seeking community employment opportunities
- Respond to crises with 24/7 on-call support, bringing stability when it's needed most

What You Bring
Education & Experience:
- Associate's degree required (Bachelor's preferred) in social work, special education, psychology, human services, or related field
- Experience with disabilities services is valued but not required—we welcome passionate newcomers to the field
- Strong communication and problem-solving skills
- Ability to work independently and as part of a team
- Proficiency with computer hardware and software programs
Essential Skills & Knowledge:
- Strong time management and ability to handle multiple priorities
- Conflict resolution and problem-solving abilities
- Commitment to maintaining confidentiality
Practical Requirements:
- Valid driver's license and reliable transportation with current auto insurance
- Ability to pass criminal background check, registry checks, and pre-employment physical including drug screening
- Willingness to obtain Ohio DODD certification (training provided)
- Ability to carry on-call phone in rotation
Work Environment: Primarily office-based with regular community visits. Phone and computer operation required daily.
