DaVita is looking for a Remote Licensed Social Worker to work with our dialysis patients. You will not be assigned to a specific location and you must be willing to get licensed in multiple states (DaVita will reimburse the cost)

What you can expect:

Schedule - Monday - Friday 9:00am - 5:00pm (some flexibility)

Social workers are responsible for the psychosocial health and wellbeing for our patients so we support those needs, providing interventions as needed and complete mandatory assessments, screenings and education

Our social workers typically have a caseload of around 115-125 patients however that's spread between clinics but may vary.

DaVita has an open position for a Licensed Social Worker who will be a vital member of each patient's core care team. For social workers we really look for empathy and good personal skills. Dialysis patients have a lot to adjust to and need a strong social worker to support them. Being able to work independently is very important but there is a lot of support on the social work side. Life on dialysis can be a difficult transition for many patients-and you'll be there to support and advocate for them. In this vital role, you will help patients understand their rights and responsibilities, and guide them in managing the physical, mental, emotional, and financial demands of End Stage Renal Disease.

Requirements:

Will need to be willing to obtain multi-state licenses (will be reimbursed)

Meets all state required regulations to practice in the dialysis setting.

All social workers must hold an LSW licensure

Master's degree in Social Work (MSW) required with a specialization in clinical practice.

Demonstrated knowledge of government and private insurance programs.

Basic computer skills in MS Word, Excel, PowerPoint, and Outlook as well as functional proficiency with DaVita specific applications within 60 days.

Salary/ Wage Range

If a candidate is hired, they will be paid at least the minimum wage according to their geographical jurisdiction and the exemption status for the position.
* New York Exempt:
*New York City and Long Island: $64,350.00/year
*Nassau, Suffolk, and Westchester counties: $64,350.00/year
*Remainder of New York state: $60,405.80/year
New York Non-exempt:
*New York City and Long Island: $16.50/hour
*Nassau, Suffolk, and Westchester counties: $16.50/hour
*Remainder of New York state: $15.50/hour
Washington Exempt: $77,968.80/year
Washington Non-exempt:
*Bellingham: $17.66/hour
*Burien: $21.16/hour
*Unincorporated King County: $20.29/hour
*Renton: $20.90/hour
*Seattle: $20.76/hour
*Tukwila: $21.10/hour
*Remainder of Washington state: $16.66/hour
