Job Description:
- Conduct comprehensive assessments of clients' needs and implement individualized care plans
- Provide case management services to individuals and families, including coordinating and monitoring services, skill development
- Collaborate with other professionals, such as social workers, counselors, and healthcare providers, to ensure holistic care
- Assist clients in accessing community resources and support services, provide transportations as needed to remove barriers
- Advocate for clients' rights and ensure their needs are met - Monitor and evaluate clients' progress towards their goals
- Provide crisis intervention and support during emergencies or challenging situations
- Maintain accurate and up-to-date documentation of client interactions and progress
- Facilitate therapeutic group
- Stay focused on the clients, the activities, and the interactions (Stay off electronic devices)
- Other duties as required
Shift: First and Second Shift, Full and Part Time
Work Schedule: M-F, 8am-5pm
Education: High School or equivalent
Wage: $18-$20 based on experience
Background/Drug Screen Policy: Must be able to pass FBI/BCI background checks, drug screen, and be eligible to work in the US.
Required Licenses or Certifications: Training will be provided
