Job Description:

  • Conduct comprehensive assessments of clients' needs and implement individualized care plans
  • Provide case management services to individuals and families, including coordinating and monitoring services, skill development
  • Collaborate with other professionals, such as social workers, counselors, and healthcare providers, to ensure holistic care
  • Assist clients in accessing community resources and support services, provide transportations as needed to remove barriers
  • Advocate for clients' rights and ensure their needs are met - Monitor and evaluate clients' progress towards their goals
  • Provide crisis intervention and support during emergencies or challenging situations
  • Maintain accurate and up-to-date documentation of client interactions and progress
  • Facilitate therapeutic group
  • Stay focused on the clients, the activities, and the interactions (Stay off electronic devices)
  • Other duties as required

 

Shift: First and Second Shift, Full and Part Time

 

Work Schedule: M-F, 8am-5pm

 

Education: High School or equivalent

 

Wage: $18-$20 based on experience

 

Background/Drug Screen Policy: Must be able to pass FBI/BCI background checks, drug screen, and be eligible to work in the US.

 

Required Licenses or Certifications: Training will be provided
