Position: Homemaker/Personal Care Professional- Weekend Warrior
Reports to: Program Manager Supervisor, Program Coordinator, Program Manager/Area Coordinator
Supervises: Individuals
Wage: $18
Shift: 1st and 2nd shifts available
Qualifications: One must have the ability to work with an interdisciplinary team, possess good interpersonal and organizational skills. Previous supervisory experience is preferred. Must have the ability to define problems, collect data, establish facts and draw valid conclusions; handle sensitive face-to-face contacts and maintain a friendly atmosphere. Must have a valid driver's license. Will be required to transport individuals for activities, doctor's appointments, etc. Must have adequate transportation to complete job responsibilities.
Basic Function: Homemaker/Personal Care Professional will develop miscellaneous support services, as stated in the ISP, including accessing and facilitating specialized and generic services, obtaining and maintaining benefits, crisis intervention, client advocacy and community education, outreach and referral responsibilities. Weekend warrior works their choice of 16 or 12-hour shifts, Friday, Saturday and Sunday only.
Primary Responsibilities:
- Personal care to the consumer in a clean and safe environment.
- Perform tasks and duties according to the ISP's, prioritize and organize tasks to achieve goals outlined on the ISP.
- Document all services provided to the consumer.
- Participate in consumer's ISP meeting if and when necessary or requested by consumer's interdisciplinary team.
- Must be sensitive to the needs of the consumer and family.
- Assurance of confidentiality of all records kept within the home.
- Attend all staff meetings.
- Assure periodic checkups and provide transportation when necessary for individuals to medical, dental, counseling appointments, and some extracurricular activities.
- Perform basic personal care and grooming, including bathing, care of hair and clothing.
- Perform household services essential to consumer's health and comfort.
- Assist with bladder and/or bowel requirements or problems, including helping consumer to and from bathroom or assisting the consumer with bedpan routines.
- Assist consumer with medications when ordered by the consumer's physician.
- Complete training as scheduled before working with individuals with developmental disabilities.
- Responsible for maintaining Red Cross First Aid training and Cardiopulmonary Resuscitation (CPR) training, and all other job-related trainings.
- Follow Horizons, Inc. policies and procedures in compliance with HIPAA Privacy Rule.
- Responsible for completion of UI’s and MUI’s and faxing them to the county board and to the Horizons office within24 hours (and guardian when applicable).
