CHC Addiction Services is a non-profit social service agency in the Akron area whose mission is to treat, inspire, support and empower individuals and families impacted by the disease of addiction. CHC has been a critical part of Ohio’s efforts to treat and prevent substance use disorders since 1974.
We are currently looking for a Case Manager for Touchstone, our Women's Residential Program. The Case Manager directs patients through substance abuse treatment through interactions with both CHC Addiction Services and in the community.
Duties Include:
- Facilitating and coordinating program enrollment of women, children and infants
- Completing appropriate case recording requirements
- Providing patients with transportation
- Providing outreach services including public speaking/presentations and public awareness activities
Minimum Requirements: Bachelor's Degree (Relevant professional experience may be considered in lieu of a degree), CDCA or other license or credential that conforms to OhioMHAS regulations pertaining to counseling services, and clean driving record.
