Description

Location: Elyria facility located at 359 Lowell St, assignments could include sites within the surrounding community as needed for Community Coaching.

Shift: This is a PRN (as-needed) position with no daily guaranteed hours. Shifts typically run Monday through Friday from 8:30 AM to 3:30 PM, though morning and afternoon flexibility is required for Non-Medical Transportation (NMT) routes, with shifts potentially starting as early as 6:30am and running until 3:30pm.

Rehabilitation Specialist Description:

Act as backup for the Elyria Rehabilitation Specialists and assist with completing community assignments to assist recent placements with job coaching efforts and potential expansion into future community programming.

Provide services to adults in vocational preparation programming. Develop activities to keep individuals active and engaged every day. If in voc prep, work with individuals to teach work skills and assist with work flow to provide continuous work opportunities.

Assist individual in engaging in programming designed to increase daily living skills and personal and social adjustment.

Shape personal and social behaviors of individuals through coaching, modeling, and demonstration.

Maintain a caseload of up to 12 individuals including case notes, ISP revision dates, documentation, recommendations for transition to next opportunities, etc. and keep supervision up to date regarding individual progress.

Assist participants with work opportunities, model appropriate behavior and training, and complete quality control measures to guarantee the work completed is accurate.

Responsible for CARF, Medicaid and agency compliance standards including completing all trainings and paperwork as assigned.

Maintain a positive work atmosphere by behaving and communicating in a professional manner that gets along with customers, vendors, individuals, co-workers, and supervisors.

NMT Driver Description:

Driver safely provides transportation for individuals with developmental disabilities to and from day services at VGS or community site while driving a non-CDL rated vehicle.

Complete daily trip check prior to each route. Note all service needs to Manager

Assists passengers on and off the vehicle (as needed) and work with residential staff or VGS staff who may be assisting to load and unload passengers

For wheelchair bound passengers, driver will secure the wheelchair to restraining devices to stabilize wheelchair during trip according to standards outlined in trainings.

Must know advance trip destination by routing / charting maps. Must follow designated route unless change is reported to Transportation Manager.

Must report to Manager via provided cellular telephone any emergencies, behavioral concerns or disruptions of service.

Must maintain the highest level of customer service with the participants at all times.

Must complete a detailed cleaning of vehicle bi-weekly and manage the servicing of the vehicle with fuel, lubricants, and accessories.
Must remove all trash and sweep (as needed) vehicle at the end of each day.

Maximize gasoline usage by following fixed routes.

Responsible for parking & securing vehicle at the end of shift.

Must record mileage and destination information in the Drivers Trip log, daily. Additionally, all insurance, safety and vehicle maintenance information will be kept in an organized and accessible manner on the vehicle.

Follows all Medicaid rules and CARF standards. Driver is responsible for maintaining all required Medicaid compliances and training.

Complete all required paperwork for Medicaid billing and submit transportation sheets to Manager

Responsible to immediately report any unusual behavior of passengers or with regards to vehicle, and follow mandatory reporting guidelines.

Qualifications



High school education and or GED plus two years experience driving; or equivalent combination of education and experience.


Drivers must have a driving record with no more than four points (representing two minor or one intermediate infraction) occurred within the last three years.


Driver must be approved by the agency driving insurance company.


Able to pass drug test.


Must pass all criminal background check requirements for Medicaid compliance.
