Position: Home Health Aide / STNA

POSITION SUMMARY

Provides basic health care tasks, hygiene services, housekeeping tasks, and supportive services essential to a client’s health and independence. Supports personal care activities including personal hygiene, ambulating, eating, dressing, shaving, and physical transfer. Performs only the activities specified in the written home health aide plan of care developed by a registered nurse or therapist.

KEY RESPONSIBILITIES

• Bathing (shower, tub, sitz, and/or bed)
• Shampoo
• Dressing and undressing
• Oral hygiene, including denture care
• Care of hearing aids, eyeglasses, prosthetic devices, and personal equipment
• Bed making (occupied and unoccupied)
• Care of incontinent clients
• Routine skin care
• Prevention of decubitus ulcers
• Perineal care
• Care of clients with a urinary catheter
• Nail care (no clipping)
• Shaving
• Obtain vital signs (temperature: oral, axillary, rectal), pulse, respiration, and blood pressure
• Measure and record intake, output, and weight
• Assist with bedpan, urinal, and commode
• Collect specimens (sputum, urine, stool
• Reinforce sterile dressings and bandages
• Change clean, non-sterile dressings for stable chronic wound
• Assist with colostomy bag changes for stable, chronic stomas
• Observe, record, and report changes in the client’s condition to the supervisor
