Position Summary

We are seeking an experienced Licensed Practical Nurse (LPN) to provide direct nursing care to individuals with developmental delays in home and community-based settings. This position requires hands-on experience with G-Tubes, J-Tubes, and tracheostomy care. The ideal candidate is dependable, professional, and committed to providing high-quality nursing support.

Key Responsibilities:

• Provide skilled nursing care based on individual service plans

• Perform and manage G-tube and J-tube feedings, medications, and maintenance

• Provide tracheostomy care, suctioning, monitoring, and emergency response

• Monitor vital signs and overall health status

• Administer medications and treatments as prescribed

• Document all care accurately and timely

• Support individuals in achieving optimal health and independence

• Communicate effectively with care teams, guardians, and supervisors

• Ensure compliance with all state regulations and company policies

Qualifications:

• Current Ohio LPN license (active and in good standing)

• Required: Experience with G-Tube, J-Tube, and trach care

• CPR/First Aid certification (or willingness to obtain)

• Ability to pass background checks and drug screening

• Reliable transportation

• Strong communication, assessment, and critical thinking skills

• Compassion, patience, and professionalism when working with developmentally delayed individuals
