Qualified Intellectual Development Professional (QIDP) - Williston, OH
Provide compassionate care and support to individuals with intellectual disabilities, ensuring they receive the necessary assistance in their daily lives. Develop and monitor service and habilitation plans; serve as the primary advocate and ensure that monitoring functions are completed appropriately. Develops necessary assessments and plans as required by procedures and standards set forth by Medicaid and other standard setting authorities. Provide appropriate training to staff regarding the above plans and procedures related to them. Facilitates various interdisciplinary team meetings to resolve concerns that arise in the ongoing evaluation, monitoring and implementation of the IHP plans; which include admission meetings, initial IHP, annual IHP, monthly/quarterly reviews, special team meetings and discharge meetings to coordinate comprehensive delivery of services for appointed caseload. Provides input and makes recommendations for policy development. Responsible for documentation and recording of minutes at various meetings. Salary starting at $25.00 per hour. 

Requirements:

• Bachelor’s Degree in Special Education, Psychology, Social Services or related field
• Strong background in patient care or case management.
• Excellent communication and interpersonal skills.
• Ability to manage conflicts effectively and compassionately.
• At least one-year experience working directly with persons with disabilities.
• Supervisory experience preferred.
