Job Description

Youth Case Manager - (Community Support Provider):
F.T., position for Youth Community Support Provider (case management) with comprehensive insurance benefits eligibility for generous paid leave, a retirement plan with employer match, productivity bonus, and EAP.

Overview:
Direct services to clients and families, including, but not limited to:  needs assessment, treatment planning, skill development, service monitoring, referral and linkage, interagency; networking, advocacy and crisis prevention.  Requires work in clients' homes, within the community and in schools.  Staff must be able to deliver services in person, by phone, and via an agency-approved telehealth platform, based on client or family preference.  Responsible for meeting established productivity standards, including billable service units in alignment with agency financial goals.  Service planning to include case coordinator and recording documentation of assessments and progress for client in achieving treatment goals.  Team/staff meetings, receiving supervision, travel and training.  Perform computer data entry.  Timely and accurate completion of time and leave reporting/forms.  Regular attendance and adherence to work schedule.  Ability to work a flexible schedule, with some evening hours required to accommodate family schedules.  E.O.E.
Minimum Requirements:
Bachelor's degree in a mental health-related field such as social work, counseling, psychology, or human/family services; no licensure or prior experience necessary.    
Degrees in related fields such as education, criminal justice, or corrections, may be considered with applicable experience. 
A high school diploma with a minimum of three (3) years of relevant experience will also be considered.
Strong written and verbal communication, assessment planning, organization skills, and teamwork are essential.  Proficiency in Microsoft Word is preferred; and ability to perform computer data entry is required.  Criminal background, drug screening, and BMV checks and clearances required.
Preferred:
Bachelor's degree in a mental health related field, as stated above, with one year paid experience, preferably with service population, and LSW.
