Job Description:
We are seeking a compassionate, detail-oriented Licensed Practical Nurse to perform in-home assessments and develop individualized care plans for clients receiving community-based services. This role is critical in ensuring that clients receive appropriate support tailored to their medical, functional, and social needs to promote independence and enhance quality of life.

Skills:
Valid LPN license in Ohio, strong assessment and care planning skills, excellent written & verbal communication, ability to work independently and manage a flexible in- home visit schedule, reliable transportation and valid driver’s license, willingness to travel to assigned service area.

Schedule:
  • Monday through Friday 
  • 1st shift
  • Full and part time opportunities
Compensation:
  • $22-$24 hr
  • 403B
  • Paid holidays, vacation, and retirement benefits 
Background check, drug screen, and valid driver's license is a condition of employment
