providing services for individuals with both alcohol/drug and mental health needs.

 

Job Description:

The Peer Navigator is a person with lived experience in recovery who uses that experience to engage, support, and empower individuals seeking behavioral health services. This position focuses on identifying and addressing social determinants of health (SDOH) barriers that may prevent individuals from engaging in or sustaining treatment. The Peer Navigator serves as a positive role model, connects clients to community resources, and promotes recovery through person-centered and strengths-based approaches.

Essential Job Functions

  • Administer SDOH screening tools with individuals seeking services at the agency.
  • Assist individuals in identifying up to three primary SDOH barriers impacting engagement in treatment.
  • Provide connections to appropriate community services to address identified barriers.
  • Ensure referrals are viable and assist in facilitating successful connections.
  • Support individuals in treatment engagement, including attending scheduled sessions.
  • Collaborate with individuals, families, and treatment providers to provide warm handoffs and ensure continuity of care.
  • Receive and respond to referrals from cross-system partners for high-risk individuals with SDOH needs or barriers to service engagement.
  • Provide outreach and active engagement, both in person and in the community, for referred individuals.
  • Share personal recovery experience appropriately to instill hope and demonstrate common understanding.
  • Use person-centered, relationship-based approaches to engage with diverse populations.
  • Complete documentation, data tracking, and outcome reporting in accordance with agency policies and procedures.

Knowledge, Skills, and Abilities

  • Lived experience with recovery and willingness to share experiences when appropriate.
  • Strong desire to help others and promote recovery.
  • Ability to serve as a positive role model for individuals in recovery.
  • Knowledge of community resources and services related to behavioral health and SDOH needs.
  • Strong communication and relationship-building skills.
  • Ability to obtain and maintain peer certification as required.
  • Commitment to client confidentiality in compliance with 42 CFR and HIPAA regulations.
  • Adherence to professional, ethical, and organizational standards of practice.
  • Ability to work independently and as part of a collaborative team.

Qualifications

  • Person with lived experience in recovery (required).
  • Eligible for or currently certified as a Peer Support Specialist/Peer Recovery Supporter or a CDCA.
  • High school diploma or equivalent (preferred). Light lifting (20 pounds or less).
  • Experience working in behavioral health or social services setting (preferred).
  • Valid driver’s license and reliable transportation (if outreach/community work is required).

Details: Starting wage is $16.22 to $17.00 per hour, based on experience, education, skills and abilities. Workdays are Monday through Friday, 8am to 5pm, 40 hours per week.
