Job Title: PASSPORT Case Manager / Waiver Service Coordinator

Pay: From $57,644.00 per year

RN or LSW in Jefferson County, Ohio

We are seeking self-motivated, energetic, detail-oriented, highly organized, tech-savvy Licensed Social Workers or Registered Nurses to join our Case Management team. Our Case Managers evaluate our members’ needs and enhance their overall wellness through home visits where we do face-to-face assessments.

Job requires travel to Jefferson County, Ohio area; therefore, applicants must have reliable means of transportation, valid driver’s license, and proof of automobile insurance.

Full-time remote position with starting salary of $57,644.

Pre-employment drug/alcohol testing and background check required.
