Family & Children First Service Coordinator Facilitator
Pay Grade/Rate of Pay: Pay Grade 5: $19.17 per hour with consideration based on experience. Plus, a comprehensive benefits package.
Family & Children First Council is currently accepting applications for full-time Family & Children First Service Coordination Facilitator position. Applicants must have good driving record and pass a drug screen.
JOB RESPONSIBILITIES: Under the general supervision of the Family & Children First Council, the Coordinator works with and serves the Council by providing Service Coordination to children/youth and families open to the Family & Children First Council office.
- Delivers Service Coordination in full compliance of the Ashtabula County Service Coordination Mechanism and to the fidelity of the model. Determines needed services, refers to services, aligns and coordinates services in place for multi-systemic youth and families; collaborates with families to develop and implement Individual Family Service Coordination Plans and create support teams, facilitates team meetings, coordinates with natural and professional supports, corresponds by phone, email, and in person.
- Writes and completes reports, correspondence, referrals, case records, other reports, documentation, and forms (e.g., Releases of Information, Individual Family Service Coordination Plans, contact/case notes, etc.). Documents all required information in the OASCIS database
- Participates in in-service training as a participant or leader and may participate in presenting Council programs and services to public or other groups. Maintains required certifications.
- Travels to case locations, relevant meetings, etc.
- Responds to public inquiries and provides routine general information in a courteous and friendly manner.
- Meets job safety requirements and all applicable OSHA safety standards that pertain to essential functions.
- Demonstrates regular and predictable attendance.
QUALIFICATIONS:
- Associates Degree in human services or related field with training and/or work experience which evidences a thorough knowledge of agency operations, working with multiple family service systems and diverse community agencies; or any equivalent combination of knowledge, skills and abilities to perform the essential functions of the position. Bachelor’s Degree in social work or related field preferred.

INHERENTLY HAZARDOUS OR PHYSICALLY DEMANDING WORKING CONDITIONS:
The employee works with and around chemicals found in an office environment (toner, correction fluid, etc.); the employee has contact with potentially violent or emotionally distraught persons; the employee may be exposed to unclean or unsanitary conditions; the employee may have exposure to potentially vicious animals; the employee may have exposure to second hand smoke; the employee may occasionally experience hostility from the general public.
*Must be able to work flexible hours when necessary, including evenings and weekends.
LICENSURE or CERTIFICATION REQUIREMENTS: Must possess a valid State of Ohio driver's license and maintain insurability under the County’s vehicle insurance policy. Must be able to successfully pass post-offer drug screen, physical and BCI fingerprint background check. Child and Adolescent Needs and Strengths (CANS) certification
