Role Overview
The Licensed Practical Nurse (LPN) Resident Care Manager supports residents living in independent living communities and clients receiving home care services. The role focuses on promoting health, wellness, autonomy, and quality of life. The LPN provides oversight of direct care services, collaborates with interdisciplinary team members, and ensures care is delivered according to established standards and within the LPN scope of practice.
Essential Duties and Responsibilities
Home Care Responsibilities
• Complete intake assessments and contribute to the development of care plans within LPN scope.
• Monitor care delivery and communicate needed updates to the care plan.
• Maintain accurate documentation, including observations, interventions, and changes in condition.
• Provide oversight and coaching to home care aides to support quality care delivery.
• Assess client appropriateness for current level of care and identify when additional services may be beneficial.
• Coordinate case management activities and make referrals to appropriate services.
• Communicate effectively with clients and families, addressing concerns and identifying solutions.
• Participate in quality improvement initiatives.
• Participate in care conferences and maintain communication with interdisciplinary team members.
• Conduct home visits for prospective clients to assess needs and determine appropriate service levels.
Independent Living Community Responsibilities
• Maintain clinic hours for residents and provide basic health counseling.
• Serve as a resource for residents by listening to concerns and offering guidance within LPN practice limits.
• Assist new residents with orientation to available care levels and collect necessary medical and contact information, including advance directives.
• Coordinate care transitions and collaborate with social workers and other team members.
• Update resident information regularly, including diagnoses, medications, physician contacts, emergency contacts, and advance directives.
• Complete pre‑admission assessments and participate in pre‑admission care conferences as needed.
Human Resource Responsibilities
• Conduct supervisory visits to evaluate competency and quality of care provided by home health aides.
Administrative Responsibilities
• Participate in after‑hours on‑call rotation; respond promptly to calls and assist with resolving care‑related concerns.
• Prepare and submit required documentation for billing.
• Maintain acceptable attendance and follow organizational policies and safety procedures.
• Perform other duties as assigned.
Behavioral Expectations
• Demonstrate professionalism, reliability, and adherence to organizational behavioral standards.
• Maintain strong interpersonal communication skills and a resident‑centered approach.
Minimum Qualifications
• Current LPN license in good standing.
• Minimum of three years of nursing experience; supervisory experience preferred.
• Experience in geriatrics, home care, or hospice is beneficial.
• Valid Ohio driver’s license.
• Strong assessment, communication, and problem‑solving skills.
• Proficiency with computer systems, including word processing and spreadsheets.
• Ability to understand and support the organization’s philosophy of care.
• Experience with basic managerial tasks such as coaching and performance feedback is helpful.
Physical Requirements
• Ability to stand and walk for extended periods.
• Ability to lift, transfer, and assist clients, including supporting up to 150 pounds with proper technique.
• Adequate vision, hearing, speech, and communication abilities.
• Ability to work in varied home environments, which may include pets, smoking, family presence, and differing physical layouts.
Working Conditions
• Office environment suitable for documentation and communication tasks.
• Frequent travel to client homes; must remain reachable by phone during scheduled hours.
• Home environments vary; safety evaluations are completed to ensure appropriate working conditions.
