Job Overview
We are seeking a professional and self-motivated Assessor. The ideal candidate will have strong interpersonal and communication skills, exceptional attention to detail, a person‑centered approach, and the ability to work independently without supervision.
In this role, you will complete comprehensive assessments using standardized InterRAI tools to support individualized care planning, service coordination, and high-quality outcomes for individuals with developmental disabilities. You will work closely with clients, caregivers, and interdisciplinary teams to ensure assessments are accurate, person-centered and aligned with regulatory standards. Applicants must have 18 months of experience conducting assessments and working with individuals with developmental disabilities. This position requires daily travel throughout Northwest Ohio, the ability to work independently with minimal oversight, and strong reliability in meeting deadlines.
This position is expected to begin in June 2026. Applicants must be available to attend required DODD training on June 9th, 10th and 11th in Columbus.
Responsibilities
- Complete InterRAI assessments in a variety of settings, including individuals’ homes, day programs, and county board locations, and in accordance with DODD guidelines for initial waiver eligibility, redeterminations or assessments completed in response to a significant change of condition for individuals enrolled in or seeking enrollment in developmental disabilities waiver services.
- Conduct other assessments on an as-needed basis.
- Communicate results of assessments with Service and Support Administrators (SSAs) to guide support planning, individual support plans, and budget determination.
- Regular daily travel within NOWAC member counties is required, though travel is generally limited to a single destination each day.
- Maintain records and documentation required per county board, Ohio Department of DD, and/or Ohio Department of Job & Family Services, or other applicable agencies.
- Work in cooperation with NOWAC and county board staff as needed during major unusual incident (MUI) investigations.
- Ensure timely and accurate documentation of services provided to individuals to support Targeted Case Management (TCM) billing.
- Maintain up-to-date knowledge of InterRAI assessment tools and procedures
Qualifications
Education and Experience
- Associate’s degree, Bachelor’s degree preferred, from an accredited college or university in any field of study. Candidates must have a minimum of 18 months of experience conducting assessments as well as experience working with individuals with developmental disabilities.
Qualifications
- Previous experience with interRAI assessment tools a plus; training available for the right candidate.
- Knowledge and ability to utilize Microsoft Office Software and systems specific to the Ohio Department of Developmental Disabilities.
- Maintains certification and ongoing competency in the use of the InterRAI assessment tool.
- Experience with office practices, procedures and operation of office equipment.
- Excellent verbal and written communication skills. Ability to organize and handle wide variety of details.
- Must be able to meet deadlines.
- Understanding and implementation of Ohio Revised Code and tools published by the Ohio Department of Developmental Disabilities.
- Applicant must be able to give professional responses and information to a variety of people, organizations, and businesses.
- Must maintain a valid Ohio Driver’s License and maintain a status of insurability for motor vehicle insurance.
- Required medical examination upon offer of employment and obtain a negative drug screen test. Successful completion of a Bureau of Criminal Investigation background check as well as other background check databases upon offer of employment.
