Summary:

The Community Resource Coordinator (CRC) is responsible for assisting patients to navigate and access community services and adopt healthy behaviors. The CRC assists the patients in addressing social determinants of health (SDOH) which include: Safe housing, transportation, education, employment opportunities, access to nutritious foods, water, and utilities. The CRC supports providers through an integrated approach with local community resources. The CRC provides social and emotional support while advocating for the patients. The CRC will also manage outbound calls in effort to ensure quality care.

Responsibilities:

  1. Responsible for establishing trusting relationships with patients while providing general support and encouragement.
  2. Provides ongoing follow-up, basic motivational interviewing and goal setting with patients.
  3. Documents phone visits and follow up phone visits
  4. Assists patients set personal health goals including maintaining scheduled appointments.
  5. Provides referrals to community agencies such as HEAP/SNAP as appropriate.
  6. Assists with obtaining and scheduling of transportation services.
  7. Collaborate with other clinical staff involved with the patients care.
  8. Documents all conversations as Telephone Encounter (TE) into the EHR.
  9. Attend staff meetings, training, as requested.
  10. Manage assigned case load of patients.
  11. Maintain excel spread sheet of patients and SDOH being addressed.
  12. Maintain HIPAA Compliance at all times

 

MINIMUM JOB REQUIREMENTS:

 

  1. High school diploma or GED certificate, required.
  2. Associate Degree in Medical Assisting, preferred.
  3. Experience may be substituted for Degree.
  4. BLS certified or achievement of certification within the first 90 days of employment - Waived

 

KNOWLEDGE, SKILLS, AND ABILITIES REQUIRED:

 

  • Ability to multi task as needed while being effective and efficient.
  • Effective oral and written communication skills.
  • Ability to work with patients from a wide diversity of social, ethnic and economic backgrounds is necessary.
  • Ability to work with a diverse group of individuals at all levels of the organization.
  • Strong communication skills are required; must be able to effectively communicate thoughts and actions through various forms of communication including verbal and written.
  • Computer literacy.
  • Ability to work with and maintain highly confidential patient information.

 

CONDITIONS OF EMPLOYMENT:

 

  • Must be willing to submit to post offer, pre-employment physical examination/medical history check if requested.
  • May be required to submit to annual TB screening.
  • A pre-employment background investigation may be required.
  • Pre-employment and random drug testing may be required.
