Position: Guardianship Services Board Case Manager

NATURE OF THE JOB

 

Job Scope:
Responsible for managing 20–30 guardianship cases appointed by the Allen County Probate Court. This position requires home, office, and telephone contact with clients, family members, and third parties; court appearances; and interventions as needed. Effectively supports ongoing and collaborative communication with community partners.

 

JOB ACCOUNTABILITIES

  1. Complete intakes to identify potential client needs, preferences, cultural and religious beliefs, views on death, and other existing supports involved in the client’s life. Refer clients to appropriate treatment and service providers. Experience in direct clinical care, courts, or a related field is required. Comply with the Allen County Guardianship Services Board Standards of Practice Handbook. 50% of time | Daily
  2. Collaborate with community agencies, including the Mental Health & Recovery Services Board of Allen County, Allen County Board of Developmental Disabilities, Allen County Job and Family Services, Lima Memorial Health System, Mercy Health St. Rita’s Medical Center, and mental health providers. Attend team meetings to facilitate person-centered guardianship decisions. 25% of time | Daily
  3. Maintain regular client contact, contacting clients monthly and more frequently based on the acuity of situations. 10% of time | Weekly
  4. Prepare court reports in a timely manner to remain in compliance with Probate Court rules and attend court hearings as required. 5% of time | Weekly
  5. Participate in working groups and/or committees to further the mission of the Board. 5% of time | Weekly
  6. Provide on-call services after hours and on weekends as needed. 5% of time | Daily

 

INTERNAL / EXTERNAL COMMUNICATIONS

  • Interacts with employees and administration of the Guardianship Services Board through face-to-face meetings, telephone, and email on a daily basis.
  • Collaborates daily with community partners, agencies, state and federal personnel, and the business community through face-to-face meetings, telephone, and email.
  • Interacts with families, individuals served, and the public through face-to-face meetings, telephone, and email on a weekly basis.

 

LEVEL OF SUPERVISION

Decisions Made Independently:

  • Routine consents from community providers
  • Treatment plans, changes of providers, and referrals
  • Completion of Probate Court forms

 

Decisions Requiring Input or Approval:

  • Adoption of Board policies
  • Amputation, inpatient surgery, and end-of-life decisions
  • Major changes in service delivery

 

SPECIAL REQUIREMENTS

 

Skills and Abilities:

  • Develop plans to identify services and supports that benefit clients
  • Effectively communicate with families and other social supports involved with clients
  • Work collaboratively with representatives of the Allen County Probate Court
  • Participate in training, consultation, and activities that enhance the Board’s mission and professional development
  • Network with other professionals in the area of guardianship
  • Meet regularly with the Executive Director to review caseloads and consult on decisions
  • Make consent decisions as a designee of the Board in accordance with internal policies
  • Perform other duties as assigned

LICENSES AND CERTIFICATES

  • Valid Ohio driver’s license with an acceptable driving record (verified annually)
  • Proof of liability insurance on a personal vehicle
  • Ohio Social Work License (preferred)
  • Must complete a six-hour guardianship fundamentals course within 30 days of hire
  • Must complete a three-hour continuing education course annually

 

TRAINING AND EXPERIENCE

  • Bachelor’s degree preferred in social work or a related field
  • One year of experience preferred in social work or similar work with guardianship services
  • Experience with computers, Microsoft Office, and standard office equipment

 

PHYSICAL REQUIREMENTS

  • Walking: 5%
  • Sitting: 80%
  • Climbing, kneeling, stooping, bending: 5%
  • Driving: 10%

 

ADDITIONAL INFORMATION

  • Must pass criminal background checks through BCII and FBI
  • Collaborate with various service providers including physicians, law enforcement, mental health agencies, and others
  • Must follow all Board policies and procedures and communicate regularly with the Executive Director
  • Position is at-will and not part of the classified civil service system or a bargaining unit
