Position Details:

• Job Title: Behavioral Health Coordinator
• Location: Weirton
• Work Schedule: Monday – Friday

Qualifications:

• Excellent organizational, critical thinking, communication, interpersonal, and multitasking skills
• Strong documentation and computer skills
• Knowledgeable in developing and implementing care plans
• Ability to quickly learn and adapt to new technology
• Ability to work effectively with diverse patients and staff while promoting a positive image for CHANGE, Inc.
• Highly self-motivated with the ability to work independently.
• Current, unrestricted CMA Certification, LPN or RN license in West Virginia and Ohio

Responsibilities:

• The role of the Behavioral Health Coordinator is to assist the Team by coordinating care to patients on the assigned provider(s) daily schedule; proactively managing and coordinating care for patients not on the schedule; and offer preventive care for all patients who are part of the assigned panel.
