Position Summary

Behavioral Health Choices (BHC) is seeking a motivated and organized Residential Case Manager to support individuals in a residential substance use disorder (SUD) treatment program. This role is responsible for managing the intake and referral process, supporting timely access to care, and assisting patients throughout their recovery journey.

The ideal candidate is self-directed, solution-focused, and committed to providing trauma-informed, person-centered care for individuals in early recovery.



Key Responsibilities

  • Receive, review, and process all referrals for residential SUD services
  • Maintain professional communication with referral sources including hospitals, detox facilities, courts, reentry programs, and community agencies
  • Conduct intake interviews and complete admission, consent, and enrollment documentation
  • Coordinate client admissions with clinical and medical staff, including Program Supervisor, SUD Counselor, and Nursing staff
  • Assist with diagnostic and clinical assessments as needed
  • Facilitate structured educational groups and life skills/recovery-based classes
  • Transport and/or accompany clients to external appointments when required
  • Complete accurate and timely documentation in the electronic health record (EHR), including assessments, treatment plans, progress notes, referrals, discharge summaries, and care coordination records
  • Ensure all documentation is completed within 24 hours of service delivery
  • Maintain expected caseload productivity (approximately 60%)
  • Participate in on-call rotation as scheduled
  • Engage in clinical supervision with a credentialed behavioral health professional
  • Support program enrollment and census goals while ensuring appropriate clinical placement and engagement


Minimum Qualifications

  • Bachelor’s degree in Social Work or related behavioral health field preferred
  • Must be eligible for or working toward one of the following credentials:
  • CDCA (Chemical Dependency Counselor Assistant)
  • QMHS (Qualified Mental Health Specialist)
  • Valid driver’s license with an insurable driving record
  • Strong organizational and documentation skills
  • Ability to manage multiple priorities in a fast-paced clinical environment
  • Proficiency with electronic health records (EHR) systems


Preferred Qualifications

  • Experience in substance use disorder treatment, case management, or residential care
  • Knowledge of trauma-informed care and recovery-oriented systems of care
  • Experience working with justice-involved or high-risk populations
  • Strong communication and coordination skills with external agencies
