Position Summary
Behavioral Health Choices (BHC) is seeking a motivated and organized Outpatient Case Manager to support patients receiving outpatient substance use disorder (SUD) and mental health services. This role is responsible for coordinating care, managing referrals, supporting patient engagement in treatment, and assisting with ongoing recovery and wellness planning in an outpatient setting.
The ideal candidate is self-directed, solution-focused, and committed to providing trauma-informed, person-centered care to patients navigating both behavioral health and substance use recovery in the community.
Key Responsibilities
- Receive, review, and process referrals for outpatient SUD and mental health services
- Maintain professional communication with referral sources including hospitals, detox facilities, mental health providers, courts, probation/parole, reentry programs, and community agencies
- Conduct intake assessments and complete required admission, consent, and enrollment documentation
- Coordinate patient engagement and scheduling with clinical staff, including counselors, supervisors, and medical personnel as needed
- Assist with screening, assessment coordination, and treatment planning support for both SUD and mental health needs
- Facilitate or co-facilitate structured educational groups, recovery groups, and life skills programming
- Provide ongoing case management services to support treatment engagement, retention, stabilization, and recovery goals for patients
- Complete documentation in the electronic health record (EHR), including assessments, progress notes, treatment updates, referrals, discharge summaries, and care coordination records
- Ensure all documentation is completed within 24 hours of service delivery
- Maintain caseload productivity expectations (approximately 60%)
- Participate in on-call rotation as scheduled
- Engage in clinical supervision with a credentialed behavioral health professional
- Support program access, engagement, and continuity of care for outpatient patients
Minimum Qualifications
- Bachelor’s degree in Social Work or related behavioral health field preferred
- Must be eligible for or working toward one of the following credentials:
- CDCA (Chemical Dependency Counselor Assistant)
- QMHS (Qualified Mental Health Specialist)
- Valid driver’s license with an insurable driving record
- Strong organizational, communication, and documentation skills
- Ability to manage multiple patients and priorities in a fast-paced outpatient environment
- Basic proficiency with electronic health records (EHR) systems
Preferred Qualifications
- Experience in outpatient mental health and/or substance use disorder treatment
- Knowledge of trauma-informed care and recovery-oriented systems of care
- Experience working with justice-involved, high-risk, or co-occurring populations
- Strong coordination skills with external agencies and multidisciplinary teams
