POSITION: Activities Coordinator (Developmental Disabilities)

POSITION PURPOSE
The position of Activities Coordinator plays a vital role in the support provided to each person in the
areas of Activities. The Activities Coordinator provides continuous and active involvement, which
includes an assertive and consistent implementation of an Individuals Service Plan, based on each
person’s strengths and needs.
The Activities Coordinator is accountable and responsible for the direct supervision and teaching - and
“hands on” assisting with each person served, including but not limited to, the following areas:
homemaker & personal care services, health & medical care, dietary & special menu orders, independent
living skill development, vocational and community employment interests, adaptive equipment and
assistance supplies/items, mobility and safety awareness, transportation, socialization & leisure skills.
The Activities Coordinator is required to function as a team member with co-workers, the interdisciplinary
team, and staff associates in an accountable and professional manner. The Direct Support Professional
will be expected to implement all job requirements in the facility and/or community setting.
The Activities Coordinator shall be responsible for all Activities Programs at PALS Chrysalis Health Adult
Day Programs. He/she shall maintain the Activities budget for all locations, assist in Activity Calendar,
and ensure scheduled activities are ran safely and properly. They are responsible for staff in the
Activities programs, including but not limited to, proper staff ratios, proper equipment, proper curriculum,
proper training.
POSITION’S ESSENTIAL FUNCTIONS
• Coordination of transportation needs to & from community events in all aspects of Activities
programming.
• Submission of daily attendance and reports changes in attendance in Activities program as
necessary.
• Assist in maintaining Activities program budgets.
• Creates and ensures meaningful Activity programing is scheduled and facilitated per the monthly
activity calendar.
• Ensure all Activity equipment is in good working order, including but not limited to, programing,
safety, mechanical, medical, etc.
• Responsible for the overall supervision of the Activities group giving support in areas of
vulnerability.
• Responsible for reviewing and ensuring all Activity documentation is completed timely and
accurately reflects the days programing. This includes, but is not limited to, transportation, billing,
and program documentation.
• Responsible for the submission of unusual incidents. This includes the assessment of risk to the
individual’s health & safety and ensures proper notification.
• Contributes to and attends weekly staff meetings.
• Promotes a supportive environment by modeling the guidelines of company philosophy,
confidentiality, people -first language, empowerment, and fair and reasonable treatment of others.
• Responsible for building and maintaining professional relationships with community vendors to
assist in Activities program planning.
Teamwork, Professionalism, and Communication
• Implements and follows company policy and procedures and information according to code of
conduct / employee handbooks and other related materials
• Assist (in cooperation with ADSM) with individuals served, and general in-service/training for
new Activity employees and other support staff as requested.
• Demonstrates professionalism within the program site and the community, in action and
appearance.
• Communicate effectively and respectfully with team members in a clear, understandable
voice, free from derogatory terminology or overtones.
• Promotes positive family, neighbor, and community employment relationships -and informs
ADSM of ALL concerns or complaints.
• Completes task/responsibilities independently and without needed additional reminders.
Support Services, Skill Development, Assistance & Supervision, and Behavior Plans:
• Responsible to implement Activity program services in a quality and in a consistent manner to
each individual served.
• Maintains and demonstrates a comprehensive knowledge of each persons Individual Service
Plan (ISP), skill development programs, behavior support plans, medications and treatments,
and general service / support guidelines and strategies.
• Monitor each individual’s personal grooming and hygiene needs, and assure that each person
is dressed appropriately for weather and occasion. Reporting concerns as necessary.
• Encourage each person’s involvement in activity planning, community integration, and
socialization.
Health and Safety
• Assist with health monitoring and prevention, reporting concerns to the appropriate person(s)
in a timely and responsible manner.
• Reports concerns with support services and programs to supervision, and makes
recommendations and shares suggestions in a team oriented manner.
• Participates in fire and emergency response drills in an efficient and safe manner.
• Demonstrates and upholds universal precautions, personal protective equipment, and
hazardous communication in a professional and responsible manner.
• Upholds safety for all by use of safety equipment and proper techniques of: back supports,
lifts, proper body mechanics, and other safety equipment as needed/required.
• Upon delegation and training/certification - ensures medication administration per doctors
orders and reports concerns / adverse reactions promptly.
• Follows current physician orders/prescriptions, current delegation standards, and applicable
medical safeguards, etc.
Staffing and Supervision:
• Demonstrates professionalism and accountability through adhering to regular work schedule,
attendance, and punctuality expectations.
• Oversees day-to-day upkeep, maintenance, and cleanliness of Activity programs/sites.
• Records days/hours worked on timecard accurately - as established per program/site.
• Obtains approval from supervisor for schedule changes, time off requests, and additional
hours in advance of occurrence - as established per program/site.
• Implements the facilitation of established Activity routines and housekeeping duties,
maintenance, and work responsibility schedules/expectations.
Financial Accounts:
• Accountable for funds and company accounts, provides safeguards for security and
monitoring, and obtains receipts for authorized transactions/purchases.
• Assist with individual funds, monies usage, receipts, signatures, in an organized and
complete manner.
Requirements:
• College degree from accredited University or equivalent there of is preferred.
• Current Drivers License with acceptable driving record is required.
• Criminal Background verification, with acceptable clearance according to applicable rules and
requirements.
• Willingness to work flexible schedule and provide responsible supervision and assistance to
adults with Developmental Disabilities.
• Ability to work as a team member and communicate in a positive and effective manner with
persons served, support staff, and others.
• Demonstrates proficiency in verbal and written skills.
• Ability to work with diverse groups and individuals.
• Maintains current and annual training requirements.
• Must successfully complete all criminal background screens as needed.
• Must be able to safely drive a company vehicle.
• Driving history must meet insurance carrier requirements.
• Maintain current certification in CPR, First Aid, Crisis Intervention, Behavior Support Plan, and
any other required training.
• Participates in personal and professional self-development to maintain or enhance work skills
that allow for completion of job responsibilities.
• Works independently, performing job functions with minimal supervision necessary.
• Other related duties, as requested or necessary.
GENERAL WORKING ENVIRONMENT
• Light to moderate traveling locally within 30 miles
• Potential exposure to violent situations and exposure to airborne/blood -borne pathogens or
other potentially infectious materials
