Position Summary:
Provides comprehensive case management services to youth enrolled in Easterseals Redwood YouthBuild.
Works with academic providers, community providers and employers, to ensure participants engage in academics and work readiness training. Assists YouthBuild participants complete their graduation requirements, paid work experiences and facilitates connections to employment and/or post-secondary education.
Job Responsibilities:
- Provides ongoing, objective, and customized assessments of the academic levels, work skill levels, and service needs of each program participant.
- Develops an Individual Opportunity Plan for each participant according to the policies and procedures established by Easterseals Redwood and the Federal Department of Labor.
- Conducts outreach and recruitment of youth for YouthBuild services.
- Identifies and arranges new or continuing services and activities that meet individual participant’s needs. This may occur when participants complete services, become disengaged, and/or face new barriers to success.
- Serves as an advocate for YouthBuild participants in securing needed services and navigating the complexities of the service delivery systems.
- Conducts training seminars and support groups for program participants, with an emphasis on programming for youth 16-24.
- Performs job development duties related to preparing program participants for employment and assisting them to connect with unsubsidized employment where possible, that is related to their training program and credentials.
- Provides supervision when required in the classroom, on field trips, and at YouthBuild worksites.
- Documents service interventions, services received and follow up to ensure services are meeting the needs of the youth and are also within the guidelines of the YouthBuild and Easterseals Redwood service models.
- Keeps accurate records by completing and maintaining required documentation including monthly case notes, monthly reports, intake forms, activity logs, supportive service and incentive logs, mileage/expense reports, etc.
- Works well with community providers and develops working partnerships that embrace the needs of youth.
- Conducts follow-up to ensure supportive service needs are addressed within the guidelines of the YouthBuild and Easterseals Redwood service model.
- Makes appropriate referrals to existing community resources and conducts follow up to ensure desired connections occur.
- Displays creativity and innovation in meeting the needs of YouthBuild participants.
- Attends meetings as required by YouthBuild Program Leadership and/or ESRW Senior Leadership.
- Maintains confidentiality of consumer files and other sensitive subject matter.
- Works assigned schedule, exhibits regular and predictable attendance and works outside of normal schedule as needed to meet workload demands. Performs other related duties as required and in a timely manner
- Conducts follow-up for twelve (12) months after each participant’s exit.
Education/Qualifications:
- An appropriate combination of education, training, course work and experience may qualify an applicant to demonstrate required knowledge, skills, and abilities.
- A valid driver’s license is required and ability to be an insured driver for Easterseals Redwood participants.
- Ability to document identity and employment eligibility within three (3) days of original appointment as a condition of employment in compliance with Immigration Reform and Control Act requirements.
- Ability to meet background check requirements and supply required documentation as outlined by Easterseals’ policies.
- Bachelor degree in human services or related field preferred.
- Experience in the residential construction process is a plus.
- Teaching and/or mentoring experience with opportunity/low income youth.
- Experience delivering services to trauma impacted youth.
