Care Coordinator (OhioRISE Care Management / Youth & Family Services)
Salary Range
$45,000 - $60,000 annually

Engage in comprehensive, ongoing assessments of a client’s ecology to determine family- and system-identified needs, strengths, resources, stressors, and sources of support to be used in treatment planning. Utilize a wraparound approach to provide intensive and/or moderate care coordination services (ICC/MCC) to clients and families as part of an OhioRISE Care Management Entity (CME) program.
✅ Essential Functions
- Align practice and work with organizational mission, vision, and guiding principles, including trauma-informed care models and wraparound values
- Provide structured service planning and care coordination through high-fidelity wraparound in accordance with national standards
- Deliver services in accordance with OhioRISE care coordination rules for ICC or MCC, including:
- Initial face-to-face contact
- Completion and updates of comprehensive CANS assessments
- Convening and facilitating Child and Family Team meetings
- Development and ongoing review of individualized care plans
- Creation of crisis safety plans
- Perform referrals and connect clients to appropriate services, including community and natural supports
- Facilitate discharge planning for youth in residential or inpatient behavioral health settings
- Support youth transitioning between facility-based and community-based care settings
- Utilize client strengths in treatment planning and intervention strategies
- Collaborate with families, caregivers, and multidisciplinary professionals to achieve positive outcomes
- Advocate for client and family needs by maintaining knowledge of community resources
- Empower families to identify needs and access support systems
- Support development of skills including behavior management, communication, social skills, and life management
- Complete and maintain required documentation in a timely manner
- Transport clients, as needed, for appointments or crisis situations
- Maintain professional licensure, if applicable
- Demonstrate proficiency in documentation systems and technology tools
- Participate in required training (including CPR, First Aid, and safety training)
- Attend team meetings, staff development, and assigned activities
- Maintain professionalism in appearance, communication, and conduct
- Work effectively with culturally diverse populations
- Demonstrate organization, time management, and teamwork
- Follow all organizational policies and procedures
✅ Required Qualifications
- Bachelor’s degree in Social Work, Sociology, Psychology, or related human services field
- OR equivalent experience (approximately 3 years in child behavioral health) may substitute
- Experience Requirements:
- Bachelor’s degree + 2 years relevant experience
- OR Master’s degree + 1 year relevant experience
- Relevant experience areas include:
- Children’s behavioral health

- Juvenile justice
- Community-based family services
- Experience and/or knowledge in:
- Family systems
- Case management
- Community resources
- Child development or counseling
- Experience working with youth experiencing:
- Trauma
- Serious emotional disturbance (SED)
- Co-occurring behavioral health conditions
- Strong communication skills (written and verbal)
- Effective decision-making and professional judgment
- Ability to work collaboratively with diverse teams and families

- Ability to meet driver eligibility requirements and transport clients when needed
