Job Description: Under the general supervision of the coordinator of Medical Services & in the complete conformance with the orders of the attending physician, the nurse will dispense medication to those individuals who require such. Nursing service will be provided in accordance with each individual’s needs who resides in the facility. Will participate as appropriate in the planning & implementation of training for the individuals. Will provide professional nursing service for individuals as indicated by the Individual Plan, based on established policies & procedures. May accept, transcribe & initiate telephone orders from the Medical Director & complete medical treatments ordered. Will complete daily nursing documentation as required in the position & assess any individual needs. Will continue professional growth by attending in-services, workshops, etc. Will order, safely administer & accurately record medications given (ICF/IID facilities). Will make nursing assessments of ill individuals, follow standing orders & notify physician if necessary. Will file paperwork for individuals if needed. Will teach CPR & First Aid classes for Horizons when applicable. Must maintain instructor eligibility. Will work all nursing hours as per staffing patterns. Will complete tube feedings as schedules & ordered. Order medication as needed. Make doctors’ appointments when necessary on your days of work. Will complete weights on Saturday/Sunday mornings & record on charts. Will trim nose hairs as needed, cut fingernails & toenails weekly. All individuals should be checked weekly. Record on chart. Check BM charts & follow up as needed. Complete Report of Major/Unusual Incident/Injury Reports as required. Implement individuals’ self-administration of medication goals & document accordingly. Other related duties.
Qualifications: Must be at least 18 years old, possess a current Ohio State Certification, Credential or Registration: LPN or higher w/ Pharmacology (at least 6 months experience in nursing preferred). Previous experience with handicapped persons desirable. Good physical & mental health a must. Must have sincere desire to work with handicapped persons. Must be willing to perform non-traditional duties. Must be well-groomed, courteous, tactful, patient, kind & pleasant when dealing with individuals. Must be level-headed in emergency situations. Must have acute sense of responsibility. Must possess a spirit of cooperation & enthusiasm. Must have a valid driver’s license. May be required to transport individuals for doctor’s appointments in a 12-passenger van with wheelchair lift.
