Responsibilities include ensuring the individual’s involvement in decisions that affect his or her care, daily schedules, and lifestyle. Coordinates all services received by an individual and assisting the individual in gaining access to needed Medicaid State planed and 1915 (i) services, as well as medical, social, educational, and other resources, regardless of funding.
Bachelor’s degree in Social Work, Counseling, Psychology, Registered Nurse or similar field required. A minimum of three years post degree experience working with individuals with severe and persistent mental illness (SPMI) and/or a minimum of one year post degree experience working with individuals with diagnosed chronic conditions.
