OhioMeansJobs|Cleveland-Cuyahoga County, OMJCC is currently sourcing Crisis Intervention Specialist candidates for local medical center.
Under the direction of the Manager of Social Work/Intake/UR/Therapy and in partnership with the senior Licensed LISW, and as a part of a multidisciplinary team the Social Worker I (BH) will provide direct services including individual and group therapeutic services to patients, including assessments, treatment plans and discharge planning. and all other duties as assigned.
Will complete psych-osocial assessments within the ODMHSAS requirement of 24 hours from the time of admission.
Creates a narrative note upon admission which describes the reason for the admission and the contacts that are to be made to ensure discharge planning begins upon admission.
In the event a patient is unable to participate with the interviewing, the Social Worker will complete the assessment by chart review and document in a progress note why the patient was unable to participate
Contacts family/case manager/guardian/POA within the first 48 hours of admission and documents in detail in the progress notes
Documents all calls to and from family in progress notes
Documents all relevant conversations to nursing homes, group homes, assisted living, rehab facilities, and all other facilities involved in the patient’s care or discharge
Meets the patients throughout the patient’s stay and documents those meetings in the progress notes
Documents anticipated discharge plan as well as updates to discharge plan in notes
Records disposition of discharge as well as method of transportation
Schedules probate hearings as necessary; to include forced medication hearings
Forwards all d/c summaries to the net provider to ensure continuity of care
Updates and individualizes patient treatment plans at discharge
KNOWLEDGE, SKILLS AND EXPERIENCE REQUIREMENTS:
Bachelor’s Degree in Social Work
LSW required; LISW, LISW-S preferred
Previous experience in behavioral health preferred
Experience working with dual diagnosis in and inpatient setting
Experience performing assessments and group counseling
Superior customer service, oral and written communication skills
Detail oriented with superior organizational skills
Ability to function well as a member of a multidisciplinary team
Effective PC skills to include MS Office, Word, Excel, and Outlook
Must be able to work flexible hours, holidays and/or evenings as needed
Must be able to effectively prioritize, facilitate and complete assignments in an accurate and timely fashion while maintaining strict confidentiality of clients/patients
Must be able to perform the essential functions of the job with or without reasonable accommodations
