Job Description: Will administer supervisory visits to clients on an intermittent basis in their place of residence. This is performed in accordance with physician orders & plan of care under the direction & supervision of the Registered Nurse. Services are furnished in accordance with Agency policies & procedures while ensuring that authorized services are in place & delivered, and that client satisfaction is achieved. All tasks must be completed in accordance with current federal, state & local standards, guidelines & regulations governing our Agency, & as may be directed by the Director of Clinical services (DCS) and/or RN Case Managers. As a LPN, you are responsible & accountable for carrying out your assigned duties as appropriate to licensure & experience. .
Qualifications: Must have a valid LPN license in the State of Ohio w/ a minimum of 1 year experience. Must possess a valid driver’s license & will be required to drive to various worksites. Must be bondable. Must be able to push, pull, move and/or lift a minimum of 30 lbs. to a minimum height of 3 feet & be able to push, pull, move and /or carry such weight a minimum distance of 50 feet. Must be bondable. Must be able to function independently, keep all client & coworker information confidential, whether acquired by word-of-mouth or on file, have personal integrity & the ability to work effectively with the clients. Must be able to work a flexible schedule, complete & turn in all necessary documentation & visit records in a timely manner, treat clients & co-workers with tact, respect & dignity, cope with the demands of the job, both mental & emotional stress, attend client care conferences as scheduled & must be able to work beyond normal duty hours, on weekends & in other positions temporarily when necessary. Is subject to call-back during emergency conditions (i.e. severe weather, evacuation, post disaster, etc.). Is subject to exposure to infectious waste, diseases, conditions etc, including TB & the AIDS & Hepatitis B viruses. Must be able to assist in the evacuation of clients during emergency situations. May be subject to the handling of the exposure to hazardous chemicals.
