Coleman Professional Services, a nationally recognized not-for-profit provider of
behavioral health and rehabilitation programs is seeking an individual to serve as the
liaison for all hospitals to be able to make a referral to Coleman for after-care services in Stark County, Ohio.
- Engage clients not attending scheduled appointments, complete ongoing assessment/monitoring of symptoms of needs of the clients served; identify barriers to attendance and address them
- Screen and evaluate consumers referred for services; screen and evaluate requests for services
- Assisting clients through community based outreaches at their homes
- Engage client in treatment and provide mental health interventions that address symptoms, behaviors by reinforcing treatment and medication adherence and coordinating follow-up with treatment providers
Qualified candidates will have a BA in related field or 3 years’ experience in mental health or a related field; a NPI number; an Ohio Medicaid number; experience working inpatient or outpatient behavioral health programs with people who are severely mentally ill; some qualifications to provide some case management for drug and alcohol disorders; familiar with community systems and agencies; and proficient with Word and Excel. Must have and maintain excellent driving record. Experience in crisis intervention preferred.
