Overview
We are seeking a part-time, Clinical Medical Social Worker for our Olentangy Primary Care Center - The Clinical Medical Social Worker is responsible for providing developmentally appropriate, outcome-focused diagnostic and therapeutic service to patients and families in accordance with the scope of practice as defined in Chapter 4757 of the Ohio Revised Code, Section 4757.01 et seq.  The Clinical Social Worker functions as a member of the interdisciplinary team and provides clinical social work services to patients and their families to improve or maintain social, emotional, functional and physical health: provides consultation, education, and training to staff regarding psychosocial issues that impact the delivery of optimal health care services; and participates in the development and delivery of education, research and continuous quality improvement. Demonstrates competencies defined (*) in Clinical Social Worker responsibilities in providing age-appropriate care to patients from the age of birth to 21 years, and some select adult patients.

What Will You Be Doing?

Facilitates the patient/family's ability to assume responsibility for managing their own health care when appropriate.
Provides relevant and timely psychosocial assessments of patients and families. Assesses the child's/family's needs for case management oversight/intervention. Collaborates with appropriate care coordinators to facilitate continuity of care and optimal health outcomes.
Provides crisis intervention as necessary to increase patient/family functioning and facilitate health management.
Provides individual, group and family brief counseling to facilitate the resolution of identified psychosocial problems to increase patient/family functioning and facilitate health management.
Promotes multidisciplinary collaboration between all members of the health care team to facilitate and coordinate quality patient care in a timely, cost-effective manner.6. Identifies and provides timely clinical social work consultation to interdisciplinary and interagency team regarding psychosocial issues related to children?s health.
Identifies, mobilizes and coordinates community and health care resources that support optimal functioning for patients and families.
Collaborates with interdisciplinary team to identify, assess, and report suspected child abuse as required by law, with particular attention to risk reduction and prevention.

Qualifications
What Are We Looking For?

To fulfill this role successfully, you must possess these minimum qualifications and experience:

Current Social Work license in Ohio required
Master’s Degree in social work required.
LISW preferred.
Two years’ experience in medical social work preferred.
Excellent customer service, communication and interpersonal skills required for interaction with all stakeholders.
Professionally appropriate analytical and organization skills required.
