Trident Clinical Care Services (TCCS) is a fast growing leader of Nurse Practitioner primary care services dedicated to senior living facility residents. We are seeking full time Nurse Practitioners interested to make a difference in health, well-being and quality of life of our senior population, and build/advance a long-lasting career. Senior Living Facilities are admitting higher acuity patients that demand top performing Nurse Practitioners to become integrated care leaders for their rehab and long term care patients. Our NPs are key providers in the patient’s collaboration team of physicians, nursing facility staff, families and other care givers dedicated to ensure and enrich each patient’s quality of life. TCCS provides our NPs with practice support and an Electronic Medical Record system which allow our providers to dedicate more time to patient care and relationships and less time with administrative tasks. TCCS provides extensive training during the onboarding phase targeted at acclimating you to the LTC setting, TCCS Clinical Programs and EMR.
