Job Description

Title: Senior RN Care Coordinator

Location: UC Health @ Victory Parkway

Department: Ambulatory

Hours: 40 hours per week

Shift: AM

Primary Purpose:

To facilitate and manage care coordination and case management across the continuum of care for UC Health patients. Working within a collaborative framework, insures the effective and efficient coordination and management of care to all patients within his/her caseload. This individual works to ensure that patients move along the health care continuum, promoting quality care, through appropriate, cost- effective interventions while maintaining close contact with patients, families, care providers, payers and community resources.

UC Health has developed a care coordination program for patients living with complex healthcare conditions that offers personalized, appropriate and supportive care aimed at enhancing patients' quality of life, reducing stress and helping patients and their caregivers navitage the healthcare system. This program is led by an experienced nurse care manager who will connect with patients directly and work collaboratively with each patient's specialists to provide the best possible care.

Knowledge and Skills

Education: Master's Science in nursing - 2 yrs, minimum required. Unencumbered registered nursing license in Ohio.

Years of Experience: Minimum of three to five years equivalent experience. minimum required.
