Description
Provide clinical pharmacy services (drug information, drugtherapy monitoring, patient-specific consultations, health care staffinservices, pharmacoepidemiology and pharmacoeconomic evaluations in Pediatricpatient populations)
Investigate/research/report adverse drug reactions
Participate in Medication Safety and Therapeutics Committeeprocess: evaluate requests for new drug usage and old formulary drug relevance,and review therapeutic classes for relevance, assist in updating hospitalformulary
Serve as an informational and patient problem solvingresource for physicians, nurses, and clinical staff pharmacists
Monitor the order writing, dispensing, and administration ofmedications.
Qualifications
Position Requirements:
Zero to five years experience as a clinical pharmacist specialist with training in drug therapy of diseases including the knowledge and application of clinical pharmacology, pharmacokinetics, drug-drug interactions, and pharmaceutics to patients OR PGY1 and PGY2 training in pediatric units
Training in cost-analysis/cost evaluation techniques preferred
Computer systems skills in spreadsheet, database, and word processing preferred
Must demonstrate highly effective written and oral communication skills
Education Requirements:
Graduate of an accredited college of pharmacy
Doctor of Pharmacy with completion of an ASHP accredited hospital or clinical residency program preferred
License Requirements:
Pharmacist licensure in the State of Ohio
Preferred Qualifications: Experience in Pediatrics highly preferred.
Availability to cover weekends and holidays preferred.
