The Neuroscience Intensive Care at UH Cleveland Medical Center is looking for an autonomous and motivated full-time night shift advanced practice provider (NP/PA) to join their team to expand night shift patient care coverage. The ideal candidate has a critical care background, comfortable working independently, competent in clinical assessment with good clinical judgement, and willing to actively engage with nursing in caring for patients. The APP will work 12- hour night shift hours from 6p-6:30a, 20 shifts in a 6-week schedule with every other weekend commitment. The APP will manage critically ill patients with neurological deficits as well as other accompanying comorbidities. Position Summary:

1.    Assessment of Health Status
Performs and documents in the electronic record the medical history and physical examination for acute and complex chronically ill patients with and without neurological injuries and deficits.
Orders and collects data using appropriate assessment techniques, relevant supporting diagnostic information and diagnostic procedures where indicated.

2.  Diagnosis
Collaborates with interdisciplinary health care team in making diagnoses of acute neurological conditions in addition to other acute care illnesses.
Manages diagnostic tests through ordering, interpretation, performance, and supervision.
Formulates differential diagnoses and prioritizes treatments appropriately.
Diagnoses complications and orders appropriate interventions.

3. Formulates Plan of Care
Identifies expected outcomes from diagnosis, and formulates and documents a plan of care to address complex acute and chronic health care needs.
Utilizes evidenced based practice guidelines or protocols in an individualized, dynamic plan of care that can be applied across the continuum.
Recognizes deviations from normal in the physical and formulates appropriate treatment plans for health problems and follow-up.

4. Implements and modifies plan of care
Prescribes diagnostic strategies and therapeutic interventions pharmacologic, non-pharmacologic and surgical, needed to achieve expected outcomes.
Writes orders for medications, blood products based upon laboratory results, routine diagnostic and follow-up studies, therapeutic measures, and post-discharge care in accordance with written practice protocols.
Manages further diagnostic tests through ordering, interpretation, performance, and supervision.
May perform advanced procedures consistent with privileges and competency validation including but not limited to: central line insertion, arterial line insertion, lumbar punctures.

5. Communication and Collaboration
Reports off to day shift team with details of a patient’s past medical history, current active issues with treatment plan and any invasive or diagnostic procedures performed during shift. Collaborates in plan of care, and follows assigned patient population throughout the shift.

Maintains ongoing communication and collaboration with interdisciplinary health care team.

Facilitates timely consults.

Facilitates communication between patient, family and staff to ensure plan of care is understood by all participants.

Assists in the management of family dynamics and coping mechanisms during acute and chronic phases of patient care. Makes appropriate referrals for supportive services.

6. Documentation and Billing.
Documents key components of patient's progress in documents such as (but not limited to): daily critical care progress note, transfer note, discharge planning profiles and discharge summaries.

Documentation is timely, meets acute care compliance standards and captures patient acuity and supports billing.

Generates billing documents for each patient and for all relevant billable procedures.

7. Professional Practice
Demonstrates Professional Practice behaviors by precepting, mentoring and educating Advanced Practice students and new Advance Practice providers to UHCMC. Participates in educational activities for nursing and resident staff.

Participates in screening eligible patients for clinical trials, maintains relevant requirements for clinical trials.

Maintains yearly requirement of 10 CEU hours of stroke education required by The Joint Commission for our Comprehensive Stroke certification.


Experience and Knowledge:
Must have current 2+ years experience in critical care as an RN or a NP/PA
Must keep current ACLS/BLS certifications
Must be willing to take Emergency Neurological Life Support Certification (ENLS).
Demonstration of quality care in prior positions.
Demonstrated leadership skills
Demonstrated skills in interpersonal relationships

Nurse Practitioner:
Nursing Doctorate or Master of Science in Nursing with emphasis on clinical practice.
Physician Assistant:
Completion of a formal Physician Assistant training from an accredited program with NCCPA certification.

Required Credentials, Licensure or Certification (i.e. RN, RRT):
Nurse Practitioner:
Current RN license to practice in Ohio
Current APRN license to practice in Ohio
Adult/Gero Acute Care Nurse Practitioner Certification
Physician Assistant:
NCCPA-certified PA (PA-C) or eligible to be certified.
Must meet all requirements of the Ohio State Medical Board for Physician Assistants.
Approval by the Ohio State Medical Board of standard or supplemental physician assistant utilization plan and supervision agreement between supervising physician and physician assistant before beginning work as a PA.
Must be credentialed by University Hospitals of Cleveland credentialing office before beginning work as a PA

Preferred Qualifications:
Experience working with Neurology and/or Neurosurgical patient population
Experienced APPs with inpatient experience in a large hospital system
