Homecare Private Duty LPN II

We are currently accepting applications for Homecare Private Duty LPNs. You must commit to work at least 16 hours a week. Up to FT may also be available. At least 1 year of nursing experience higly preferred.
Math test given at time of interview. Sample test and answer key will be provided beforehand. Calculator is permitted.
Pay range: $19.50 to $21.25 per hour plus shift differential

One week mandatory nursing orientation.
Further orientated and trained by a RN Preceptor until fully competent.
RN and Scheduler on call 24 hours/7 days per week to provide support.

Provides direct continuous skilled nursing care or skilled nursing visits for individuals and/or groups of acute, chronically ill and terminal patients as prescribed by NCH and community physicians in compliance with regulations as established by the State Board of Nursing, Medicare/Medicaid guidelines, JC standards, and the policies and procedures of Homecare and NCH to patients in the home setting or other environments as approved. The care provided by the NCHH LPN shall reflect the highest standards of nursing practice and shall be provided in a manner which promotes quality, continuity and safe patient care. Responsible for providing care for pediatric patients and selected adult patients. Demonstrates competence defined (*) NCHH LPN responsibilities in providing age-appropriate care and service to patients from the age of birth to 21 years, and selected adults.

1. *Follows the patient plan of care, as ordered, authorized and scheduled, under the direction of the Homecare Patient Care Coordinator (PCC) (RN).
2. * Promotes and provides necessary patient/caregiver patient education for prescribed therapies.
3. * Correctly calculates, prepares, and administers medications by the five rights of medication administration.
4. Initiates purposeful, written and verbal communication with patient/family, and other members of the health care team to convey nursing actions, patient/family response, and progress toward goals during routine as well as stressful situations.
5. * Implements continuous nursing care or skilled nursing visits for pediatric patients/families in sometimes stressful environments, demonstrating competence in nursing care according to established NCHH/NCH standards and protocols.
6. Demonstrates sound clinical decision-making in collaboration with the Homecare Patient Care Coordinator and/or NCHH RN.
7. Provides thorough, clear and concise documentation of care provided in accordance with established guidelines.
8. * Reviews utilization of service and patient progress with the Homecare Patient Care Coordinator and/or NCHH RN Provides input into the revision of the plan of care based upon data collected and patient/family needs.
9. *Provides instructions and explains procedures to patients/families according to established standards.
10. *Ensures that appropriate patient care protocols and quality controls are in place for the delivery, reporting, follow-up, and evaluation of home nursing services.
11. Develop/finalize/revise/update/terminate service agreements as appropriate.
12. Assure complete documentation for billing purposes and match practice with authorized allowables with patient needs.
13. Provide initial/ongoing advocacy, customer service, and communication with family and all customers
14. Need/ability to balance cost, quality, and mission.
15. Maintains confidentiality of all patient/agency information.
16. Assumes responsibility for continued professional growth.
17. Possess knowledge of medical terminology, office equipment, different service lines/roles, documentation requirements, Information system, utilization review, referral process/sources, discharge process, care planning, use of internal/external resources, applicable regulatory requirements, community resources, chart process, scheduling, resource availability, payer coverage policies, medical equipment, ability to provide patient education as appropriate to practice, and basic knowledge of diagnoses and procedural coding, as appropriate.
18. Performs basic skills in the following: phone communication, computer skills (word processing and data entry), communication, organizational/multi-tasking, customer service, team building, supervising staff as appropriate, management/leadership skills as appropriate, negotiation, conflict management, decision making, and diplomacy skills.
19. Performs other duties as assigned.
JOB SPECIFIC RESPONSIBILITIES:
LPN-I 1. *Following additional education, training and demonstrated competency, selected LPN Is may perform procedures related to the care of tracheostomy and ventilator patients, as well as perform venipunctures for the purpose of drawing blood only.LPN-II 2. *Consistently accepts assignments and consistently performs care for pediatric patients requiring tracheostomies and ventilators. Following additional education, training and demonstrated competency, selected LPN II?s may perform venipunctures for the purpose of drawing blood only.
1. Must be a graduate of an approved school of practical or vocational nursing.
2. Must be licensed as a practical nurse in the state of Ohio.
3. CPR certification required.
4. Strong working knowledge of the concept of family centered care.
5. Must have the ability to function collaboratively within the framework of an Interdisciplinary Team.
6. Must demonstrate problem solving, organization, communication skills and ability to work independently and react appropriately to crisis situations.
JOB SPECIFIC QUALIFICATIONS:
LPN-I 1. Must have an interest in the pediatric and homecare setting. LPN-II 1. Must have experience and knowledge in the care of medically fragile children, specifically G/J tube care and tracheostomy care, and a beginning knowledge of ventilator care and maintenance.2. One year of experience in a pediatric care setting and one year of home health experience preferred.Home Visit LPN 1. One year previous pediatric home health experience preferred.2. Two years experience in a pediatric acute care setting preferred.
