Both positions are part-time, working 32 hours a week, day shift with 3-5 on-call nights/month.

Minimum $27.25/hr

The Hospice/Palliative Care Nurse is responsible for providing patient/family home care in compliance with regulations as established by the State Board of Nursing, State Licensure Regulations, Medicare/Medicaid Guidelines, JC Guidelines, and the policies and procedures of Nationwide Children's Hospital Homecare (NCHH) and Nationwide Children's Hospital, Inc. (NCH). The Hospice/Palliative Care Nurse coordinates the implementation of the interdisciplinary plan of care for each patient/family. The Nurse actively participates in hospice/palliative care program team meetings and provides on-going education and training programs for NCHH and NCH staff on hospice/palliative care. The Nurse demonstrates competence defined (*) in Principle Duties and Responsibilities in providing age-appropriate care and service to patients from the age of birth to select adults.

1. Assesses the appropriateness of patients and families for entry into the Hospice/Palliative Care Program.

2. Provides direct nursing care to appropriate patients/families.

3. Ensures appropriate assessment, planning, implementation and evaluation of patient and family services in conjunction with the hospice/palliative care team members under the direction of the Hospice Program Manager.

4. Treats the patient as an individual by striving to meet his/her physical, psychological, emotional, spiritual and social needs; addresses child growth and developmental issues.

5. Collaborates with the interdisciplinary team and the family in the development of appropriate family-centered care plans following the Medicare Conditions of Participation and other regulatory guidelines.

6. Attends and participates in the biweekly Interdisciplinary Team (IDT) meetings as needed to assist in the evaluation and review of services being provided to each patient and family according to Medicare / Medicaid Hospice Conditions of Participation (COP) and state licensure requirements.

7. Assists patients and families in establishing appropriate/attainable goals and developing plans to meet those goals prior to their discharge from hospital to home. Reviews goals as needed to ensure they remain appropriate.

8. Participates in planning patient visits with the interdisciplinary team (i.e., RN, Social Worker, Chaplain, etc.).

9. Implements home infusion and nursing care per established policies and procedures and physician's written orders.

10. Provides supervision of the home health or hospice aide when utilized in the care of the palliative/hospice patient.

11. Collaborates with agencies, professional groups and others to facilitate the provision of coordinated, interagency services for children and families receiving hospice/palliative care services.

12. Contributes to or provides professional educational programs regarding nursing issues involved in care of children and their families in the hospice/palliative care programs, especially as they relate to issues of life-limiting conditions.

13. Participates in orientation and training of staff and volunteers and hospital staff in-services.

14. Provides thorough, clear, and concise documentation of care management provided in accordance with NCHH policy, JC standards, Medicare / Medicaid Conditions of Participation, and clinical nursing care standards.

15. Completes required documentation of services utilizing the point of care information system in accordance with NCHH policy and Medicare COP to facilitate accurate and timely billing and payroll processing.

16. Shares on-call responsibilities with the Hospice/Palliative Care Team on an equitable basis.

17. Pursues educational endeavors that will support continued professional growth for self and team.

18. Identifies issues in practice that warrant increased investigation in order to better serve the needs of the Hospice/Palliative Care patients and participates in relevant research activities that will provide information necessary for more effective, efficient, collaborative delivery systems.

19. Promotes, models and participates in NCH and Homecare customer service initiatives; fosters patient / parent / caregiver, payer, provider and other community relationships through using effective communication and collaboration skills; and openly and willingly provides staff and managers with customer feedback, in an appropriate and goal-oriented way.

20. Actively participates in NCHH continuous quality and performance improvement activities under the direction of the QI Manager and respective manager(s) (including event tracking and reporting, patient case conferences, performance improvement teams, customer satisfaction surveys, utilization review activities, etc.)

21. Conducts self in a manner that supports good patient relations and projects a professional image consistent with NCH and NCHH standards.

22. Maintains at all times the rights of the patient / family regarding confidentiality of the medical record, according to policy.

23. Supports the philosophy, mission, policies, procedures, standards, and strategic goals. Participates, as directed, in short and long-range planning and Homecare strategic goal development, implementation and evaluation.

24. Manages own learning and skills related to existing and new computer systems, as appropriate, including becoming proficient in NT / Outlook and Microsoft Office.

25. Participates in general and department staff meetings, committee work and other related activities as assigned.

26. Participates in marketing efforts and in community endeavors providing education regarding Hospice/Palliative Care and Homecare services.

27. Performs other duties as assigned, including the assumption of new duties commensurate with the growth of the agency.

28. Balances cost, quality and mission in all decision-making.



- Graduate of an accredited school of nursing; BSN preferred. Current, valid Ohio nursing license.

- Two years experience and skill in the nursing care of children with life-threatening diseases.

- Willingness to accept new responsibilities commensurate with the growth of the Hospice/Palliative Care program. Knowledge of Hospice/Palliative Care and/or Homecare and pain management preferred.

- Strong problem-solving, organizational, leadership and communication skills. Well-developed coping mechanisms.

- Ability to work independently and react appropriately to crisis situations.

- Valid Ohio driver's license, proof of automobile insurance and a clean driving record.

- Clean background check.

- Specific interest in caring for patients with terminal or life-limiting conditions.

- Experience preferred. Experience with line care, dressing changes, NG tubes, and other high-level nursing skills strongly preferred.

- Communication and organizational skills needed for care coordination of complex patients required.
