Position Summary/Essential Duties:
-Provide Clinical Pharmacy Services (drug information, drug therapy monitoring, patient and caregiver education regarding medications, resolution of various medication-related issues, pharmacoepidemiology and pharmacoeconomic evaluations, and decision support in critically ill patients with neurological conditions).
-Primary clinical responsibilities will be to identify, evaluate, and assist in the management of medication-related problems throughout the neurosurgical intensive care and acute care stay.
-Investigate/research/report adverse drug reactions in this patient population.
-Participate in Pharmacy and Therapeutics Committee process: evaluate requests for new and old drug usage and formulary changes for this patient population
-Serve as an informational and patient problem solving resource for staff clinical pharmacists
Experience & Knowledge:
0-5 years experience as a pharmacist with training in drug therapy of critically ill patients including the knowledge and application of clinical pharmacology, pharmacokinetics, drug-drug interactions, and pharmaceutics to patients; training in cost-analysis/cost evaluation techniques preferred
Doctor of Pharmacy with completion of an ASHP accredited hospital or clinical residency program
Required Credentials, Licensure or Certification (i.e. RN, RRT): Pharmacist licensure in the State of Ohio
Interest in ICU/ER highly preferred.
