Professional and Personal Qualifications:

Ø  Successful completion of Home Health Aide or Nurse Aide training program and certification.

Ø  Maintain good physical health and mental health including Tb testing (refer to Health Screening policy).

Ø  Possess interpersonal skills required to work with others.  Ability to explain patient care procedures, answer questions and calm patient fears, as well as be able to communicate information on patient condition to nursing and/or therapy staff.

Ø  Demonstrate integrity, good judgment, initiative, and analytical skills required to perform various tasks as assigned.  Must be able to follow directions accurately and in a timely manner.

Primary Duties/Responsibilities:

1.    Provide safe personal care to patients in their home. Including but not limited to: Bathing, shampoos, skin and foot care, mouth care and perineal care, emptying and rinsing ostomy appliances and external urinary devices, positioning, ROM exercises, turning, positioning and ambulating patients, transferring patients using proper equipment and proper body mechanics, simple unsterile dressing changes, assisting with feeding and light meal preparation, assistance with medications: self-administered medications, and incidental/ household tasks during personal care/ service visits.

2.    Reporting observed changes in patient condition and contributing to care plan by communicating information to primary nurse/therapy staff.

3.    Communicates orally and in writing with primary nurse/therapist and/or supervisor, i.e., reports all patient problems, patient needs and other pertinent information for care in accord with agency criteria.

4.    Demonstrates and documents safe, quality personal care and meeting third party reimbursements related to personal care services.

5.    Documents appropriately all patient-related communications, attempted home visits, and other pertinent information to the case manager.

6.    Timely responsiveness to all agency customers with excellent service behaviors of personalized attention, courtesy and compassion.

7.    Obtains and documents patient care information in an accurate and timely manner on the home health aide progress notes, i.e., temperature, pulse, respiration, intake/output, height/weight, etc...

8.    Maintaining patient’s rights and a general understanding of patients physical, psychosocial, and developmental care needs.

9.    Utilizing infection control measures and universal precautions in accord with agency policy/procedures.

10. Responding appropriately to emergencies and utilizing emergency procedures in accord with agency policy/procedures.

Physical/Environment Demands:

1.    Must be able to hear, see, speak, sit, stand, bend and climb stairs, stoop, and lift 50-75 lbs.

2.    Must be adaptable to a variety of home/community setting.

3.    Must be able to travel/drive in all types of weather.
