The Charge Nurse is responsible for performing a variety of duties on assigned unit during the second or third shifts to deliver nursing care and within scope of practice coordinates care delivery which will ensure residents' needs are met in accordance with professional standards of practice through physician orders, facility policies and procedures and federal, state and local guidelines; coordinating with the Director of Nursing and the Nursing Unit Manager to establish specific goals; for determining work procedures and expediting work flow; for ensuring compliance with all operating policies and procedures and Ohio Department of Health regulations and Federal Health Care Administration regulations.
Essential Functions Statement(s)

Greets newly admitted residents upon admission; escorts them to rooms as necessary
Participates in orientation of new residents/family members to community
Informs nursing personnel of new admissions, expected time of arrival, room assignment, etc.; ensures rooms are ready for new admissions
Assesses residents on admission to develop interim care plan and further assesses in depth to contribute to the complete care plan; updates care plans as warranted and required; attends and participates at plan of care meetings as scheduled
Assesses residents and analyzes obtained information to develop appropriate interventions to prevent decline; records signs of change in condition; notifies appropriate staff to evaluate weight loss, decline in ambulation, development of skin breakdown and other changes; observes conditions that indicate possible need for restorative nursing programs and then notifies appropriate staff of the need for evaluation; informs the physician of changes in assessment when appropriate
Communicates status of residents to incoming personnel according to established procedures; communicates with physicians; transcribes, clarifies and implements physicians' orders
Distributes and administers prescribed medications and treatments, when required, according to approved nursing protocols, including IVs if certified
Maintains accountability for controlled substances; inventories medications at the beginning and end of the shift with another licensed nurse; notifies supervisor of discrepancies
Supports the facility's medical supply system; follows procedures for resident charge items
Ensures that an adequate supply of floor stock medications, supplies and equipment is on hand to meet the nursing needs of the residents; reports needs to Nursing Unit Manager
Notifies Nursing Unit Manager or Nursing Unit Supervisor of all drug and narcotic discrepancies noted on assigned shift
Reviews medication records for completeness of information and accuracy in the transcription of the physician's order, and adherence to stop order policies
Notifies the attending physician of automatic stop orders prior to last dosage being administered
Disposes of drugs and narcotics as required, and in accordance with established procedures
Administers professional services such as; catheterization, tube feedings, suction, applying and changing dressings/bandages, packs, colostomy, and drainage bags, takes blood, gives massages and range of motion exercises, cares for dead/dying, etc., as required
Obtains sputum, urine and other specimens for lab tests as ordered
Takes and records TPRs, blood pressures, etc., as necessary
Responds to emergencies; assesses the condition of the resident; decides on the type of medical attention required; calls appropriate ambulance service if needed; notifies family when resident is transferred or admitted to the hospital; notifies the physician when a change in condition occurs
Makes rounds to ensure care delivery meets quality standards and restorative protocols as needed
Confers with residents and families; makes referrals to other departments when appropriate
Maintains resident clinical files; keeps charts updated; documents
Completes employee and resident incident report forms; completes follow-up investigation forms
Completes accident/incident reports as necessary
Correlates accurate and complete documentation and its impact on reimbursement and federal regulations
Maintains the Daily Census Report and submits to office as required
Completes and files required record-keeping forms/charts upon the resident's admission, transfer and/or discharge
Charts nurses' notes in an informative and descriptive manner that reflects the care provided to the resident, as well as the resident's response to care
Fills out and completes transfer forms in accordance with established procedures
Reports all discrepancies noted concerning physician's orders, diet change, charting errors, etc., to the Director of Nursing and/or Nursing Unit Manager
Ensures that residents are aware of the services available to them; encourages residents to participate in activity programs and provides staff support to enable residents to get to and from activities and to participate in them
Cooperates with other resident services when coordinating nursing services to ensure that the resident's total regimen of care is maintained
Records new or changed diet orders; forwards information to Dining Services department
Checks foods brought by family members, ensuring it complies with residents' orders, dietary needs and safety
Arranges for diagnostic and therapeutic services, as ordered by physician, and in accordance with established procedures
Consults with resident's physician in providing resident's care, treatment, rehabilitation, etc., as necessary
Reports absentee call-ins to the Nursing Unit Manager
Ensures residents who are unable to call for help are checked on frequently
Meets with residents, and/or family members, as necessary; reports problem areas to the Nursing Unit Manager or Supervisor
Assists in planning the nursing services portion of the resident's discharge plan
Reviews and participates in the updating of the resident's written discharge plan
Assists in arranging transportation for discharged residents as necessary
Reports and initiates investigations on all allegations of resident abuse and/or misappropriation of resident property
Performs all other duties as assigned or directed

Skills & Abilities

Education: Graduate of a state-accredited and approved licensure program: Required
Experience: One (1) year of experience in nursing/long-term care, preferred
Computer Skills: Must be able to proficiently operate a computer, the Internet and basic office equipment
Certifications & Licenses: Successful completion of a state-approved pre-licensure nursing education program; Active LPN license conferred by the State Board of Nursing; Must obtain and maintain mandatory, state and federal requirements and certifications for practice or occupation; Must be CPR certified
Other Requirements: Good base knowledge of pharmacology, treatments, issues specifically concerning the elderly, dementia, various diseases, and side effects; Demonstrated people skills, with special attention to interfacing with patients" family members; Working knowledge of different cultures and religions; Must be familiar with/able to use the following equipment: WiFis, pagers, point click care, computer, sit to stand, hoyer, gait belt; Must be able to read, write, understand and speak the English language; Special care/dementia care employees will have additional training in caring for and treating dementia residents; Must be able to work with elderly residents, families and other health care staff members; Must be able to adhere to all terms and conditions set forth in the United Church Homes Employee Handbook.
